數(shù)值模擬不同充氣壓力對(duì)支架術(shù)后頸動(dòng)脈力學(xué)環(huán)境的影響

【摘要】:目的研究支架介入在不同充氣壓力下對(duì)病變血管壁力學(xué)環(huán)境的影響。方法使用高分辨率磁共振成像(MRI)獲得含有脂質(zhì)斑塊和鈣化斑塊的頸動(dòng)脈模型,通過有限元軟件構(gòu)建體內(nèi)血管-支架兩者之間相互作用的力學(xué)模型,數(shù)值模擬分析不同充氣壓力(909、1 212和1 515 kPa)作用下血管壁及斑塊的Von Mises應(yīng)力分布。結(jié)果隨著充氣壓力的增大,血管壁與支架接觸內(nèi)表面的Von Mises應(yīng)力逐漸增大。在高充氣壓力作用下支架撐開后,脂質(zhì)斑塊區(qū)域會(huì)產(chǎn)生嚴(yán)重的應(yīng)力集中現(xiàn)象;而在不同充氣壓力作用下,鈣化斑塊區(qū)域Von Mises應(yīng)力分布差異很小。結(jié)論較大充氣壓力作用下支架撐開后將給血管壁和脂質(zhì)斑塊帶來較大傷害,可能會(huì)引發(fā)支架后期失效。研究結(jié)果可為支架術(shù)充氣壓力的選擇以及支架植入后斑塊穩(wěn)定性的預(yù)估提供臨床參考。
[Abstract]:Objective to study the effect of stent intervention on the mechanical environment of diseased vascular wall under different inflatable pressure. Methods High resolution magnetic resonance imaging (MRI) was used to obtain carotid artery models with lipid plaque and calcified plaque. A mechanical model of the interaction between vascular and stent was constructed by finite element software. The Von Mises stress distribution of vascular walls and plaques under different inflatable pressures (909KPA, 1 212 and 1 515 KPA) was analyzed numerically. Results with the increase of inflatable pressure, the Von Mises stress on the inner surface of vascular wall and stent was gradually increased. When the scaffold was opened under high inflatable pressure, the stress concentration in lipid plaque region would be serious, but the stress distribution of Von Mises in calcified plaque region had little difference under different inflatable pressure. Conclusion when the stent is opened under the action of inflatable pressure, the vascular wall and lipid plaque will be damaged, which may lead to the later failure of the stent. The results can provide clinical reference for the selection of inflatable pressure and the prediction of plaque stability after stent implantation.
