【摘要】：目的研究毛母細(xì)胞瘤(TB)的病理形態(tài)特征、診斷及鑒別診斷。方法回顧性分析3例TB臨床病理特點(diǎn)、形態(tài)學(xué)及免疫表型特征,以及與多種疾病的鑒別診斷;另選擇5例基底細(xì)胞癌行免疫表型檢測(cè)并重點(diǎn)與TB相鑒別診斷。結(jié)果鏡下可見,3例中腫瘤均位于皮下,不與表皮相連,單一的基底樣細(xì)胞構(gòu)成腫瘤主體,腫瘤周邊細(xì)胞呈柵欄狀排列。例1可見大小不等的囊腔及色素沉積,診斷為色素性TB;例2可見原始毛乳頭樣結(jié)構(gòu),診斷為TB;例3腫瘤中的基底樣細(xì)胞核呈柵欄狀排列,形成波紋狀結(jié)構(gòu),診斷為波紋狀TB。3例雄激素受體(AR)及2例B淋巴細(xì)胞2(Bcl-2)陰性表達(dá),1例細(xì)胞角蛋白(CK)20散在陽性,3例CD10間質(zhì)及毛乳頭樣結(jié)構(gòu)陽性;5例基底細(xì)胞癌周邊柵欄狀排列的腫瘤細(xì)胞CD10陽性,4例AR陽性,3例Bcl-2陽性,5例CK20陰性。結(jié)論 TB是來源于毛囊生發(fā)上皮的良性腫瘤,完整切除預(yù)后良好,需重點(diǎn)與基底細(xì)胞癌相鑒別。
[Abstract]:Objective To study the pathological features, diagnosis and differential diagnosis of blastomas (TB). Methods The clinical and pathological features, morphology and immunophenotypic characteristics of 3 cases of basal cell carcinoma and the differential diagnosis of multiple diseases were analyzed retrospectively. Results Under the microscope,3 cases of the tumors were located under the skin and not connected with the epidermis, and the single basal-like cells formed the tumor main body, and the peripheral cells of the tumor were in a fence-like arrangement. example 1, the sac cavities and the pigment deposits of different sizes were found, and the diagnosis was pigmentary TB; in case 2, the original hair papilla-like structure was seen, and the diagnosis was TB; the substrate-like nucleus in the tumor of the third tumor was arranged in a fence shape to form a corrugated structure, 3 cases of androgen receptor (AR) and 2 cases of B-lymphocyte 2 (Bcl-2) negative expression,1 case of cytokeratin (CK)20 were positive,3 cases of CD10 were positive, and 3 cases of CD10 were positive. The CD10 positive,4 AR-positive,3-Bcl-2-positive and 5-CK20-negative were found in 5 cases of basal cell carcinoma. Conclusion TB is a benign tumor derived from hair follicle germinal epithelium.
