【摘要】：目的觀察改良拖入式輸尿管腸管吻合在惡性膀胱腫瘤患者原位回腸膀胱術(shù)中的應(yīng)用效果。方法收集惡性膀胱腫瘤患者79例,均行膀胱根治性切除術(shù)+原位回腸膀胱術(shù);其中37例(A組)采用腸管外黏膜下隧道式輸尿管腸管吻合方法進行原位回腸膀胱術(shù),42例(B組)采用改良拖入式輸尿管腸管吻合方法進行原位回腸膀胱術(shù)。結(jié)果兩組均手術(shù)順利,A組吻合輸尿管與腸管所需時間為(27.6±3.4)min,B組為(5.2±0.5)min;兩組相比較,B組吻合輸尿管與腸管所需時間短于A組(P㩳0.05)。A組術(shù)后并發(fā)吻合口瘺2例、吻合口狹窄3例、尿液反流2例、尿路感染2例,并發(fā)癥發(fā)生率為24.32%;B組術(shù)后并發(fā)吻合口瘺1例、尿路感染2例,并發(fā)癥發(fā)生率為7.14%。兩組并發(fā)癥發(fā)生率比較,P㩳0.05。結(jié)論在惡性膀胱腫瘤患者原位回腸膀胱術(shù)中,改良拖入式輸尿管腸管吻合方法能明顯縮短手術(shù)時間,降低術(shù)后泌尿系統(tǒng)并發(fā)癥的發(fā)生。
[Abstract]:Objective to observe the effect of modified drag ureter-intestinal anastomosis in situ ileal cystectomy in patients with malignant bladder tumor. Methods 79 patients with malignant bladder tumor were treated with radical cystectomy and orthopaedic ileal cystectomy. In group A, 37 cases (group A) underwent in situ ileal cystectomy with extraintestinal submucous tunnel ureter-intestinal anastomosis, and 42 cases (group B) underwent in situ ileal cystectomy with modified towed ureter-intestinal anastomosis.
