【摘要】：目的觀察膀胱不同部位注射A型肉毒素治療女性難治性膀胱過度活動癥的療效。方法 40例入選病例,隨機分為逼尿肌注射組(A組)、逼尿肌聯(lián)合三角區(qū)注射組(B組),評估患者治療前及治療后4周的臨床癥狀(平均每日日間排尿次數(shù)、平均每次排尿量、夜尿次數(shù)、尿急次數(shù))及尿動力學指標(初始尿意膀胱容量、最大膀胱容量)和OABSS評分、QOL評分情況。結果經(jīng)過A型肉毒素注射治療4周后,A組平均每日日間排尿次數(shù)、平均每次排尿量、夜尿次數(shù)、尿急次數(shù)以及OABSS評分,QOL評分、最大膀胱容量均較B組有明顯改善(P0.05)。結論 A型肉毒素膀胱內注射治療難治性膀胱過度活動癥療效好,無明顯毒副作用,是治療難治性膀胱過度活動的經(jīng)濟、有效的新方法;采用逼尿肌聯(lián)合三角區(qū)部位的注射,癥狀緩解更為明顯,值得進一步的研究及推廣。
[Abstract]:Objective to observe the effect of botulinum toxin type A injection into different parts of bladder in the treatment of refractory bladder hyperactivity in women. Methods 40 patients were randomly divided into detrusor injection group (group A) and detrusor combined with triangular injection group (group B). The clinical symptoms of the patients before and 4 weeks after treatment (average daily urination times) were evaluated. Average urination volume, nocturnal urine frequency, urination urgency times), urodynamic indexes (initial urinary bladder volume, maximum bladder volume), OABSS score, QOL score. Results after 4 weeks of botulinum toxin A injection treatment, the average daily urination times, average daily urination volume, nocturnal urine frequency, urination urgency times, OABSS score and QOL score in group A were average daily urination times, average urination volume per day, nocturnal urine frequency, urination urgency times, OABSS score and QOL score. The maximum bladder volume was significantly improved compared with group B (P 0.05). Conclusion intravesical injection of botulinum toxin A is effective in the treatment of refractory bladder hyperactivity without obvious toxic and side effects. It is an economical and effective new method for the treatment of refractory bladder hyperactivity. The symptom relief is more obvious by injection of detrusor combined with triangular area, which is worthy of further study and promotion.
