【摘要】：目的：探討膀胱鱗癌與浸潤(rùn)性尿路上皮癌在發(fā)病特點(diǎn)、治療及預(yù)后的異同點(diǎn)，增強(qiáng)對(duì)膀胱鱗癌的進(jìn)一步認(rèn)識(shí)，提高診治水平。 方法：回顧性分析我院2011年1月至2014年1月收治的膀胱鱗癌9例、浸潤(rùn)性尿路上皮癌32例的臨床資料，并參考國(guó)內(nèi)外相關(guān)文獻(xiàn)進(jìn)行討論分析。 結(jié)果：9例膀胱鱗癌和32例浸潤(rùn)性尿路上皮癌病例中，在血尿?yàn)槭装l(fā)癥狀、疾病進(jìn)展中發(fā)生肉眼血尿不具有統(tǒng)計(jì)學(xué)意義(P0.05)。而在表現(xiàn)為膀胱刺激癥狀、尿常規(guī)中白細(xì)胞明顯增多、腫瘤臨床分期、腫瘤是否單發(fā)的差異具有統(tǒng)計(jì)學(xué)意義(P 0.05)。膀胱鱗癌組，在手術(shù)方式不同情況一年后生存率和一年內(nèi)死亡率間的差異不具有統(tǒng)計(jì)學(xué)意義(P0.05)，但整體死亡率較高。 結(jié)論：膀胱鱗癌在臨床上較為少見，癥狀無(wú)特異性，早期診斷較為困難，且浸潤(rùn)性較強(qiáng)，惡性程度很高，確診時(shí)多數(shù)患者已發(fā)展至晚期，預(yù)后與腫瘤的臨床分期有緊密的相關(guān)性，因此早期的臨床診斷和合理的治療極為重要。
[Abstract]:Objective: to investigate the differences and similarities between bladder squamous cell carcinoma and invasive urothelial carcinoma in the pathogenesis, treatment and prognosis of bladder squamous cell carcinoma, and to enhance the understanding of bladder squamous cell carcinoma and improve the level of diagnosis and treatment. Methods: the clinical data of 9 cases of bladder squamous cell carcinoma and 32 cases of invasive urothelial carcinoma treated in our hospital from January 2011 to January 2014 were analyzed retrospectively. Results: in 9 cases of bladder squamous cell carcinoma and 32 cases of invasive urothelial carcinoma, hematuria was the first symptom. However, there were significant differences in the symptoms of bladder irritation, leukocytosis in urine routine, clinical stage of tumor and whether the tumor was single or not (P 0.05). In the bladder squamous cell carcinoma group, there was no significant difference between the survival rate and the mortality rate within one year in different operation conditions (P0.05), but the overall mortality rate was higher. Conclusion: bladder squamous cell carcinoma is rare in clinic and has no specific symptoms. Early diagnosis is difficult, invasive and malignant. Most of the patients have developed to the late stage by the time of diagnosis. The prognosis is closely related to the clinical stage of the tumor, so early clinical diagnosis and reasonable treatment are very important.
