【摘要】：目的探索利用自制膀胱測(cè)壓預(yù)警系統(tǒng)實(shí)施低壓經(jīng)尿道前列腺電切術(shù)(TURP)的可行性與安全性。方法 2014年1月-2016年1月167例行前列腺電切的前列腺增生(BPH)患者按隨機(jī)數(shù)字表法分為測(cè)壓組(A組)和非測(cè)壓組(B組)。A組85例,采用經(jīng)皮膀胱造瘺+TURP術(shù)式42例(A1組),連續(xù)沖洗鞘式TURP術(shù)式43例(A2組)。B組82例,采用經(jīng)皮膀胱造瘺+TURP術(shù)式42例(B1組),連續(xù)沖洗鞘式TURP術(shù)式40例(B2組)。A組利用自制虹吸管經(jīng)膀胱穿刺形成膀胱測(cè)壓預(yù)警系統(tǒng)進(jìn)行膀胱測(cè)壓,實(shí)時(shí)監(jiān)測(cè)膀胱壓力,保持膀胱低壓狀態(tài)進(jìn)行TURP手術(shù)。4組均檢測(cè)術(shù)前、術(shù)后血Na~+,記錄術(shù)中手術(shù)時(shí)間、術(shù)中出血量、切除前列腺質(zhì)量、尿色轉(zhuǎn)清時(shí)間,評(píng)估手術(shù)前后的國(guó)際前列腺癥狀評(píng)分(IPSS)、最大尿流率(Qmax)、殘余尿量(PVR)和生活質(zhì)量評(píng)分(QOL)。結(jié)果 A組血Na~+術(shù)前術(shù)后差異無(wú)統(tǒng)計(jì)學(xué)意義,B組血Na~+術(shù)前術(shù)后差異有統(tǒng)計(jì)學(xué)意義,提示測(cè)壓組較非測(cè)壓組更安全。測(cè)壓組A1與非測(cè)壓組B1、測(cè)壓組A2與非測(cè)壓組B2相比可用于切除前列腺組織的安全手術(shù)時(shí)間更長(zhǎng)、前列腺切除質(zhì)量更多,術(shù)后尿色轉(zhuǎn)清時(shí)間更短,Qmax、IPSS改善更佳,提示測(cè)壓組較非測(cè)壓組治療效果更佳。結(jié)論自制膀胱測(cè)壓預(yù)警系統(tǒng)可以及時(shí)發(fā)現(xiàn)TURP術(shù)中膀胱內(nèi)高壓狀態(tài),可以及時(shí)處理造成高壓的原因,始終能保持低壓沖洗下進(jìn)行TURP,延長(zhǎng)了安全手術(shù)時(shí)間,增加組織切除率,明顯降低電切綜合征的發(fā)生,使TURP更具安全性。
[Abstract]:Objective to explore the feasibility and safety of low pressure transurethral resection of prostate (TURP) by using self-made bladder pressure-warning system. Methods from January 2014 to January 2016, 167 patients with benign prostatic hyperplasia (BPH) were randomly divided into two groups: group A (n = 85) and group B (n = 85). 42 cases (group A1) were treated with percutaneous cystostomy (TURP), and 82 cases (group A 2) with continuous rinsing sheath type TURP (group A 2) and group B (n = 82). 42 cases (group B1) of percutaneous cystostomy (group B1) and 40 cases (group B2) of continuous washing sheath type TURP (group A) were used to measure bladder pressure by using self-made siphon to form bladder pressure-warning system through bladder puncture, and to monitor bladder pressure in real time. Before and after operation, the blood Na ~, the time of operation, the amount of blood loss, the quality of prostatectomy and the time of urine color change were recorded in the group of TURP operation with maintenance of low pressure of bladder. Assessment of preoperative and postoperative international prostatic symptom scores (IPSS), maximum uroflow rate (Qmax), residual urine volume (PVR) and quality of life score (QOL). Results there was no significant difference in blood Na ~ before and after operation between group A and group B. there was significant difference between group B and group A, suggesting that the blood Na ~ in group A was more safe than that in group B. Group A1 and group B _ 1, group A _ 2 and group B _ 2 can be used to remove prostate tissue for longer safe operation time, prostate resection quality more, postoperative urine color conversion time is shorter and Qmax-IPSS improved better, compared with non-pressure group B2, the pressure group A2 can be used to remove prostate tissue more safely, the quality of prostatectomy is more, and the time of urine color conversion is shorter after operation. The results suggest that the treatment effect of the manometry group is better than that of the non-manometry group. Conclusion the self-made bladder pressure-warning system can detect intravesical high pressure in TURP in time, deal with the causes of high pressure in time, keep low pressure irrigation for TURP all the time, prolong the safe operation time and increase the rate of tissue resection. The incidence of electrocution syndrome was significantly reduced, and TURP was more safe.
