【摘要】：目的:了解呂梁市人民醫(yī)院泌尿外科門(mén)診就診的良性前列腺增生(BPH)患者下尿路癥狀(LUTS)的特征、分布及嚴(yán)重程度。方法:對(duì)2016年04-10月份期間在呂梁市人民醫(yī)院泌尿外科門(mén)診就診的50歲以上的在臨床上確診為BPH且愿意接受調(diào)查的男性患者進(jìn)行相關(guān)檢查及下尿路癥狀問(wèn)卷調(diào)查。問(wèn)卷調(diào)查包括國(guó)際前列腺癥狀評(píng)分(IPSS評(píng)分)和生活質(zhì)量評(píng)分(QOL評(píng)分)。結(jié)果:478例BPH患者中51-60、61-70、71-80、80歲各年齡組例數(shù)分別為77(16.11%)、129(26.98%)、154(32.22%)、118(24.69%)。前列腺平均體積為38.06±13.78ml,51-60、61-70、71-80、80歲各年齡組前列腺平均體積分別為32.1±9.476ml、34.89±14.83ml、39.94±12.70ml、42.90±14.21ml,各年齡組見(jiàn)前列腺體積差別有統(tǒng)計(jì)學(xué)意義(P0.05),且隨年齡組的增長(zhǎng)前列腺體積增大。478名BPH患者中中、重度LUTS癥狀患者共432例,占總數(shù)的91.38%。51-60、61-70、71-80、80歲年齡組患者中中、重度LUTS癥狀例數(shù)分別為62(82.82%)、113(87.60%)、145(94.16%)、111(94.07%)。QOL評(píng)分平均4.20±1.39,不太滿意、苦惱、很糟(QOL評(píng)分為4、5、6分)的患者共352例,占總數(shù)的73.64%。478名BPH患者中以急性尿潴留就診者共76例,占BPH患者的15.90%,51-60、61-70、71-80、80歲各年齡組例數(shù)分別為5(6.58%)、17(22.37%)、26(34.21%)、28(36.84%)。各年齡段急性尿潴留發(fā)生率之間的差別有統(tǒng)計(jì)學(xué)意義(P0.05),且年齡越高尿潴留發(fā)生率越高。結(jié)論:BPH)患者在泌尿外科門(mén)診患者中所占比例很高,其中中、重度LUTS癥狀的患者所占的比例很高,生活質(zhì)量較差,反應(yīng)出了當(dāng)?shù)谺PH患者就診意識(shí)差、就診時(shí)間晚。需要對(duì)中老年BPH/LUTS患者進(jìn)行健康普查和宣教,提高患者就診意識(shí),早做臨床干預(yù),提高患者生活質(zhì)量。
[Abstract]:Objective: to investigate the characteristics, distribution and severity of lower urinary tract symptoms (LUTS) in patients with benign prostatic hyperplasia (BPH) in Urology outpatient of Luliang people's Hospital. Methods: from April to October 2016, the male patients over 50 years old who had been diagnosed with BPH in clinic and were willing to be investigated were examined and the lower urinary tract symptoms were investigated in the Urology Clinic of Luliang people's Hospital from April to October 2016. The questionnaire included International Prostate symptom scale (IPSS) and quality of Life score (QOL). Results among the 478 cases of BPH, the number of patients aged 51-6061-7071-80 and 80 years old were 77 ~ 16.11 and 129-26.98, respectively. The mean volume of prostate was 38.06 鹵13.78ml / 51-6061-7071-80 and 32.1 鹵9.476ml / d 34.89 鹵14.83ml respectively. The difference of prostate volume between different age groups was statistically significant (P0.05N), and the prostate volume increased with the increase of prostate volume in the age group (.478 patients), and the mean volume of prostate was 39.94 鹵12.70 ml / d (42.90 鹵14.21 ml) in each age group, and the prostatic volume increased with the increase of prostatic volume in the age group (P < 0.05), and the mean volume of prostate was significantly higher than that in the control group (P < 0.05). There were 432 patients with severe LUTS symptoms, accounting for 91.38.51-600.61-7071-80 years of age. The number of patients with severe LUTS symptoms was 6222.822n 117.60. The average score was 4.20 鹵1.39. 352 patients were not satisfied, distressed, and had a very bad QOL score of 45, 6). Of the 73.64.478 BPH patients, 76 were diagnosed with acute urinary retention, accounting for 15.90% of the patients with BPH. The number of patients aged 71-80 years old was 56.58%. The number of patients with acute urinary retention was 176.58%, 2634.21% and 2836.84%, respectively. There were significant differences in the incidence of acute urinary retention among different age groups (P 0.05), and the higher the age, the higher the incidence of urinary retention. Conclusion the proportion of LUTS patients in urological outpatients is very high, among them, the proportion of patients with severe LUTS symptoms is very high, the quality of life is poor, which reflects the poor consciousness of local BPH patients and the late visit time. It is necessary to carry out general health survey and education for middle and old age patients with BPH/LUTS, improve their consciousness of seeing doctors, make early clinical intervention, and improve their quality of life.
