【摘要】：目的探討臨床藥師參與腎移植術后患者的藥學監(jiān)護。方法臨床藥師參與1例腎移植術后患者的藥學監(jiān)護,通過開展基因檢測、完善治療方案、提出藥學建議、進行患者教育等為患者提供個體化的藥學服務。結果與結論患者病情好轉出院。對腎移植術后患者的藥學監(jiān)護,應著重于免疫抑制藥的治療方案、其他合并癥的治療、用藥教育與生活方式指導等。
[Abstract]:Objective to explore the pharmacological monitoring of clinical pharmacists after renal transplantation. Methods Clinical pharmacists took part in the pharmacological monitoring of one patient after renal transplantation, and provided individualized pharmaceutical care through gene detection, improvement of treatment plan, pharmaceutical advice and patient education. Results and conclusion the patients were discharged from hospital. The pharmacological monitoring of renal transplantation patients should focus on the treatment of immunosuppressive drugs, other complications, medication education and lifestyle guidance.
