【摘要】：目的分析重型顱腦損傷(SBI)患者尿路感染病原菌分布及危險(xiǎn)因素分析。方法選取醫(yī)院2014年8月-2016年8月收治的SBI患者156例作為研究對象,對所有納入患者的術(shù)后尿路感染情況進(jìn)行觀察和分析,對與患者術(shù)后尿路感染發(fā)生相關(guān)的因素進(jìn)行研究和分析,對發(fā)生尿路感染患者尿液樣本中的病原菌進(jìn)行檢測和分析。結(jié)果術(shù)后發(fā)生尿路感染患者51例,感染率32.69%,感染患者以發(fā)熱、尿路刺激征、尿道口瘙癢為主要臨床體征;共分離病原菌72株,其中革蘭陰性菌35株占48.61%,以大腸埃希菌為主;革蘭陽性菌11株占15.28%,以糞腸球菌為主,真菌26株占36.11%,以白色假絲酵母菌為主;合并糖尿病、GCS評(píng)分、留置導(dǎo)尿管時(shí)間是SBI患者發(fā)生尿路感染的獨(dú)立危險(xiǎn)因素(P0.05)。結(jié)論 SBI術(shù)后尿路感染的相關(guān)因素、臨床體征和病原菌分布具有特征性,給予充分的重視和有效的診治,以提高SBI患者救治成功率和醫(yī)院感染工作的效果。
[Abstract]:Objective to analyze the distribution and risk factors of urinary tract infection in patients with severe craniocerebral injury (SBI). Methods 156 patients with SBI admitted from August 2014 to August 2016 were selected as the study subjects, and the postoperative urinary tract infection of all the patients involved were observed and analyzed. The factors related to postoperative urinary tract infection were studied and analyzed. The pathogenic bacteria in urine samples of patients with urinary tract infection were detected and analyzed. Results there were 51 patients with urinary tract infection after operation, the infection rate was 32.69. The main clinical signs were fever, urinary tract irritation sign and urinal pruritus. 72 strains of pathogenic bacteria were isolated, of which 35 strains were Gram-negative bacteria (48.61%), mainly Escherichia coli. There were 11 Gram-positive bacteria (15.28%), mainly Enterococcus faecalis, 26 fungi (36.11), and most of them were Candida albicans. GCS score and indwelling catheter time were the independent risk factors of urinary tract infection in SBI patients (P 0.05). Conclusion the related factors, clinical signs and pathogenic bacteria distribution of urinary tract infection after SBI are characteristic, which should be paid enough attention to and effective diagnosis and treatment, in order to improve the success rate of treatment of SBI patients and the effect of nosocomial infection.
