【摘要】：目的探討螺旋狀套石籃輔助輸尿管軟鏡鈥激光碎石術(shù)(FURL)治療單側(cè)直徑1.0 cm輸尿管上段結(jié)石臨床療效及安全性。方法研究對象選取該院2012年1月-2015年12月收治的單側(cè)直徑1.0 cm輸尿管上段結(jié)石患者共140例,以隨機抽簽法分為對照組(70例)和觀察組(70例),分別采用FURL術(shù)單用和在此基礎上加用螺旋狀套石籃輔助治療;比較兩組患者圍手術(shù)期臨床指標、碎石成功率、結(jié)石清除率、結(jié)石上移率及術(shù)后并發(fā)癥發(fā)生率等。結(jié)果觀察組患者手術(shù)時間明顯長于對照組,差異具有統(tǒng)計學意義(P0.05);兩組患者術(shù)中出血量和住院時間比較差異無統(tǒng)計學意義(P0.05);觀察組患者碎石成功率和結(jié)石清除率均明顯高于對照組,差異具有統(tǒng)計學意義(P0.05);觀察組患者結(jié)石上移率明顯低于對照組,差異具有統(tǒng)計學意義(P0.05);同時兩組患者術(shù)后并發(fā)癥發(fā)生率比較差異無統(tǒng)計學意義(P0.05)。結(jié)論螺旋狀套石籃輔助FURL術(shù)治療單側(cè)直徑1.0 cm輸尿管上段結(jié)石可有效提高結(jié)石清除效果,降低結(jié)石上移風險,且未增加術(shù)后并發(fā)癥發(fā)生概率。
[Abstract]:Objective to evaluate the efficacy and safety of helical lithotripsy assisted with holmium: YAG laser lithotripsy in the treatment of unilateral upper ureteral calculi (1.0 cm in diameter). Methods 140 patients with upper ureteral calculi with a diameter of 1.0 cm were selected from January 2012 to December 2015. Randomly divided into control group (n = 70) and observation group (n = 70) were treated with FURL alone or on the basis of spiral stone basket, the clinical indexes of perioperative period, lithotripsy success rate, stone removal rate were compared between the two groups. The rate of upward removal of stones and the incidence of postoperative complications. Results the operative time of the patients in the observation group was longer than that in the control group. The difference was statistically significant (P 0.05), there was no significant difference in blood loss and hospitalization time between the two groups (P 0.05). The success rate of lithotripsy and the removal rate of stone in the observation group were significantly higher than those in the control group. The difference was statistically significant (P 0.05), the rate of stone upward movement in the observation group was significantly lower than that in the control group (P 0.05), and there was no significant difference in the incidence of postoperative complications between the two groups (P 0.05). Conclusion Spiral stone basket combined with FURL in the treatment of upper ureteral calculi with unilateral diameter of 1.0 cm can effectively improve the removal of stones, reduce the risk of stone upward migration, and do not increase the probability of postoperative complications.
