【摘要】：目的分析慢性腎臟病(CKD)5期患者的高血壓相關(guān)危險(xiǎn)因素。方法收集390例合并有高血壓病的CKD5期患者的基本資料,根據(jù)性別、年齡、原發(fā)病、透析方式、體質(zhì)量指數(shù)(BMI)、合并癥(高血脂、高尿酸、心功能不全)、文化程度、血甲狀旁腺激素(PTH)水平進(jìn)行單因素分析;將單因素分析得出的有統(tǒng)計(jì)學(xué)意義的變量納入Logistic多因素回歸分析,篩選出CKD 5期患者高血壓控制的危險(xiǎn)因素;并分析高血壓分段評(píng)估的降壓藥物限定每日劑量(DDD)。結(jié)果該研究高血壓總體達(dá)標(biāo)率為22.8%。單因素分析結(jié)果顯示:年齡40歲、男性、糖尿病腎病、高血壓腎病、血液透析、高脂血癥、高尿酸、高PTH均具有統(tǒng)計(jì)學(xué)意義(P0.05,P0.01);Logistic多因素回歸分析顯示,糖尿病腎病、高脂血癥、高PTH是CKD 5期患者高血壓的獨(dú)立危險(xiǎn)因素。在高血壓的分段評(píng)估中,1段和0段的DDD差異無統(tǒng)計(jì)學(xué)意義(P0.05),2段、3段的DDD較0段和1段明顯升高(P0.05)。結(jié)論 CKD 5期患者高血壓的整體達(dá)標(biāo)率較低;糖尿病、高脂血癥、PTH是CKD 5期患者高血壓的獨(dú)立危險(xiǎn)因素。
[Abstract]:Objective to analyze the risk factors of hypertension in patients with chronic kidney disease (CKD) stage 5.Methods the basic data of 390 CKD5 patients with hypertension were collected according to sex, age, primary disease, dialysis mode, body mass index (BMI), complications (hyperlipidemia, hyperuric acid, cardiac insufficiency, education level).The serum parathyroid hormone (PTH) level was analyzed by univariate analysis, and the statistical variables obtained by univariate analysis were incorporated into Logistic multivariate regression analysis to screen out the risk factors for hypertension control in patients with stage 5 of CKD.The daily dose limit of antihypertensive drugs was analyzed.Results the overall compliance rate of hypertension in this study was 22. 8%.Univariate analysis showed that age of 40 years, male, diabetic nephropathy, hypertension nephropathy, hemodialysis, hyperlipidemia, hyperuric acid, high PTH were all statistically significant. Logistic multivariate regression analysis showed that diabetic nephropathy, hyperlipidemia, diabetes nephropathy, hyperlipidemia.High PTH is an independent risk factor for hypertension in patients with stage 5 CKD.There was no significant difference in DDD between stage 1 and segment 0 in the segmental assessment of hypertension. The DDD in the third segment of P0.05 was significantly higher than that in stage 0 and stage 1.Conclusion the overall achievement rate of hypertension in patients with CKD 5 is low, and diabetes mellitus and hyperlipidemia are independent risk factors of hypertension in stage 5 of CKD.
