【摘要】：目的:比較鑲嵌式唇黏膜和膀胱黏膜在治療尿道下裂手術(shù)后復(fù)雜性尿道皮膚瘺手術(shù)成功率和并發(fā)癥發(fā)生率。方法:對(duì)符合手術(shù)指征的55例尿道下裂術(shù)后復(fù)雜性尿道皮膚瘺患者,隨機(jī)分成唇黏膜取材組(36例)和膀胱黏膜取材組(19例),于術(shù)中分別應(yīng)用自體游離唇黏膜和膀胱黏膜行尿道成形術(shù),并評(píng)估手術(shù)并發(fā)癥和術(shù)后尿道皮膚瘺的再發(fā)率。結(jié)果:鑲嵌式唇黏膜和膀胱黏膜在治療復(fù)雜性尿道皮膚瘺術(shù)后,均有2例出現(xiàn)尿道皮膚瘺,1例出現(xiàn)尿道狹窄,手術(shù)成功率分別為91.7%、84.2%,無(wú)統(tǒng)計(jì)學(xué)差異(P0.05);術(shù)后陰莖外形美觀。兩組術(shù)后并發(fā)癥發(fā)生率也無(wú)統(tǒng)計(jì)學(xué)差異(11.1%vs 26.3%,P0.05)。結(jié)論:鑲嵌式唇黏膜和膀胱黏膜修復(fù)復(fù)雜性尿道皮膚瘺均取得了理想的效果,但唇黏膜修復(fù)供材區(qū)創(chuàng)傷更小,手術(shù)簡(jiǎn)單快捷,值得在臨床上推廣應(yīng)用。
[Abstract]:Objective: to compare the surgical success rate and the incidence of complications in the treatment of complicated urethral fistula after hypospadias surgery with inlaid lip mucosa and bladder mucosa. Methods: 55 cases of complicated urethral skin fistula after hypospadias surgery were treated according to the surgical indications. The patients were randomly divided into two groups: the lip mucosa group (n = 36) and the bladder mucosa group (n = 19). Urethroplasty was performed with free lip mucosa and bladder mucosa respectively during the operation. Results: there were 2 cases of urethral skin fistula and 1 case of urethral stricture after the treatment of complicated urethral skin fistula. The successful rate of operation was 91.7% and 84.2%, respectively. There was no statistical difference between the two groups (P 0.05). There was no significant difference in postoperative appearance of penis between the two groups. There was no significant difference in the incidence of postoperative complications between the two groups. Conclusion: the repair of complex urethral skin fistula with inlaid lip mucosa and bladder mucosa is not significant. With the ideal effect, But the injury of labial mucosa repair donor area is less, the operation is simple and quick, and it is worth popularizing in clinic.
