【摘要】：目的 了解NLRP3、IL-18在IgA腎病患者腎活檢組織標本中的表達,進一步分析NLRP3、IL-18與臨床病理指標、MCP-1表達的關系,從而探尋三者在IgA腎病中的作用。 方法 選取2013年8月至2013年11月入住我院并經(jīng)臨床和腎活檢病理確診的原發(fā)性IgA腎病病人50例,健康對照組共5例。收集所有研究對象的臨床資料并分組,分為單純血尿組,單純蛋白尿組,血尿和蛋白尿組。根據(jù)病理改變,分為局灶節(jié)段硬化組和輕微病變組。運用免疫組織化學方法分別測定腎組織中NLRP3、IL-18和MCP-1的表達,用圖像分析法分別對其進行半定量。 結果 1.NLRP3、IL-18和MCP-1在Ign腎病腎組織表達情況 結果顯示NLRP3、IL-18和MCP-1主要見于腎小管上皮細胞胞漿和間質(zhì)單核淋巴細胞,對照組小管間質(zhì)基本不表達,IgA組小管間質(zhì)表達顯著增加,差異有統(tǒng)計學意義(P0.05)。 2.NLRP3、IL-18和MCP-1在IgA腎病不同臨床分型腎組織中的表達 (1)NLRP3、IL-18和MCP-1在IgA組表達較正常對照組有顯著差異(P0.05)。 (2)NLRP3、IL-18和MCP-1在單純蛋白尿組、血尿蛋白尿組表達較單純血尿組有顯著差異(P0.05)。 3.NLRP3、IL-18和MCP-1在IgA腎病不同病理分型腎組織中的表達 (1)局灶節(jié)段硬化組和輕微病變組NLRP3、IL-18和MCP-1表達存在顯著差異性(P0.01)。 (2) NLRP3、IL-18和MCP-1表達與腎小管萎縮、腎間質(zhì)炎癥細胞浸潤及腎間質(zhì)纖維化呈正相關。 4.Ign腎病腎組織中NLRP3、IL-18與臨床指標、MCP-1表達的關系 (1) NLRP3、IL-18和MCP-1表達與尿蛋白呈正相關,與肌酐、尿素氮等無明顯相關性。 (2)相關分析顯示,NLRP3、IL-18表達在臨床分組和病理分組中均與MCP-1表達呈正相關,NLRP3表達在臨床分組和病理分組中均與IL-18表達呈正相關。 結論 1.IgA組與對照組比較,小管間質(zhì)中NLRP3、IL-18和MCP-1表達明顯增加,提示以上指標可能參與IgA腎病的發(fā)生發(fā)展。 2.局灶節(jié)段硬化組和輕微病變組NLRP3、IL-18和MCP-1表達存在顯著差異性,表明NLRP3、IL-18和MCP-1可能與腎臟病理損害程度相關,其可以成為判斷IgA腎病預后的指標。 3.IgA腎病中NLRP3、IL-18和MCP-1與蛋白尿、腎小管萎縮、腎間質(zhì)炎癥細胞浸潤及腎間質(zhì)纖維化呈正相關,提示以上指標參與IgA腎病蛋白尿的發(fā)生以及小管間質(zhì)的損傷。 4.NLRP3、IL-18和MCP-1表達存在正相關。提示IgA腎病中,MCP-1升高可能趨化NLRP3,使NLRP3表達增高,進一步導致下游IL-18增加,進而參與小管間質(zhì)損傷。
[Abstract]:Purpose. To investigate the expression of NLRP3- IL-18 in renal biopsy specimens of patients with IgA nephropathy, and to further analyze the relationship between NLRP3- IL-18 and the expression of MCP-1, and to explore the role of NLRP3- IL-18 in IgA nephropathy. Method. Fifty patients with primary IgA nephropathy admitted to our hospital from August 2013 to November 2013 and confirmed by clinical and renal biopsy were selected, and 5 healthy controls were selected. The clinical data of all the subjects were collected and divided into simple hematuria group. Simple proteinuria group, hematuria group and proteinuria group were divided into focal segmental sclerosis group and mild lesion group according to pathological changes. The expression of NLRP3IL-18 and MCP-1 in renal tissue was detected by immunohistochemical method. The method of image analysis was used for semi-quantitative analysis. Results. 1. Expression of IL-18 and MCP-1 in renal tissue of Ign nephropathy. The results showed that IL-18 and MCP-1 were mainly found in the cytoplasm and interstitial monocyte of renal tubule epithelial cells, while in the control group, the tubulointerstitial expression was significantly higher than that in the control group (P 0.05). 2. Expression of IL-18 and MCP-1 in different clinical types of IgA nephropathy. The expression of IL-18 and MCP-1 in IgA group was significantly higher than that in normal control group (P 0.05). The expression of IL-18 and MCP-1 in NLRP3 and MCP-1 was significantly higher in the proteinuria group than in the hematuria group (P 0.05). 3. Expression of IL-18 and MCP-1 in different pathological types of IgA Nephropathy. (1) there was a significant difference in the expression of IL-18 and MCP-1 between the focal segmental sclerosis group and the mild lesion group. (2) the expression of IL-18 and MCP-1 in NLRP3 was positively correlated with renal tubular atrophy, interstitial inflammatory cell infiltration and interstitial fibrosis. 4. The relationship between NLRP3mIL-18 and the expression of MCP-1 in renal tissue of patients with Ign nephropathy. 1) the expression of IL-18 and MCP-1 in NLRP3 was positively correlated with urinary protein, but not with creatinine and urea nitrogen. (2) correlation analysis showed that the expression of IL-18 was positively correlated with the expression of MCP-1 in clinical and pathological groups. The expression of NLRP3 was positively correlated with the expression of IL-18 in both clinical and pathological groups. Conclusion. 1. The expression of IL-18 and MCP-1 in tubulointerstitium in IgA group was significantly higher than that in control group, suggesting that the above indexes might be involved in the occurrence and development of IgA nephropathy. 2. There were significant differences in the expression of IL-18 and MCP-1 between the focal segmental sclerosis group and the mild lesion group, indicating that NLRP3IL-18 and MCP-1 may be related to the degree of renal pathological damage, which can be used as a prognostic index of IgA nephropathy. 3. The levels of IL-18 and MCP-1 in IgA nephropathy were positively correlated with proteinuria, renal tubular atrophy, interstitial inflammatory cell infiltration and renal interstitial fibrosis, suggesting that the above indexes were involved in the development of proteinuria and the injury of tubulointerstitial tissue in IgA nephropathy. 4. There was a positive correlation between IL-18 and MCP-1 expression in NLRP3s, which suggested that the increase of MCP-1 in IgA nephropathy may chemotaxis NLRP3, increase the expression of NLRP3, further increase IL-18 downstream, and then participate in tubulointerstitial injury.
