【摘要】：目的探討老年人群24 h動(dòng)態(tài)血壓變異性與腎功能損害的關(guān)系。方法在參加開灤研究隊(duì)列第三次健康體檢的人群中,采取整群抽樣的方法隨機(jī)抽取3064例年齡≥60歲的開灤集團(tuán)離退休員工,對(duì)其進(jìn)行動(dòng)態(tài)血壓監(jiān)測(cè),最終納入統(tǒng)計(jì)分析的為1382例。根據(jù)動(dòng)態(tài)收縮壓的標(biāo)準(zhǔn)差(SSD)四分位數(shù)將調(diào)查人群分為四組,測(cè)定各項(xiàng)生化指標(biāo),計(jì)算腎小球?yàn)V過率(eGFR),采用秩相關(guān)分析和逐步線性回歸模型分析SSD與eGFR的關(guān)系。結(jié)果在1382例觀察對(duì)象中,平均年齡為67.16±5.86歲,其中,男性905例(65.5%),女性477例(34.5%)。24h SSD、日間SSD和夜間SSD均與eGFR呈負(fù)相關(guān);對(duì)e GFR的影響因素進(jìn)行逐步線性回歸分析,校正其他混雜因素后,24h SSD、日間SSD仍與e GFR呈負(fù)線性相關(guān)。結(jié)論 24h SSD和日間SSD與eGFR呈負(fù)線性相關(guān)。
[Abstract]:Objective to investigate the relationship between 24 h ambulatory blood pressure variability and renal dysfunction in elderly people. A cluster sampling method was used to randomly select 3064 retired employees of Kailuan Group, aged more than 60 years, and to monitor their ambulatory blood pressure. 1382 cases were included in the statistical analysis. According to the quartile number of SSDs of dynamic systolic blood pressure, the investigated population was divided into four groups, and the biochemical indexes were measured. The relationship between SSD and eGFR was analyzed by rank correlation analysis and stepwise linear regression model. Results the mean age was 67.16 鹵5.86 years. In 905 cases of male, 65.5% of them, 477 cases of female had negative correlation with eGFR, day SSD and night-time SSD were negatively correlated with eGFR, and the influencing factors of eGFR were analyzed by stepwise linear regression analysis. After adjusting for other confounding factors, there was still a negative linear correlation between daytime SSD and eGFR after 24 h SSD.Conclusion 24 h SSD and daytime SSD have negative linear correlation with eGFR.
