依托咪酯全憑靜脈麻醉對老年骨科患者術(shù)中應(yīng)激反應(yīng)和早期術(shù)后認(rèn)知功能障礙發(fā)生率的影響

【摘要】:目的:探討全憑靜脈麻醉下應(yīng)用依托咪酯維持麻醉后老年骨科患者術(shù)后認(rèn)知功能障礙(POCD)的發(fā)生情況,闡明依托咪酯對老年骨科全麻患者術(shù)中應(yīng)激反應(yīng)和POCD發(fā)生率的影響。方法:選取2012年5月—2013年1月吉林大學(xué)第一醫(yī)院行骨科手術(shù)的老年患者91例(年齡60歲,ASA分級(jí)Ⅰ~Ⅲ級(jí)),采用隨機(jī)數(shù)字表法分為依托咪酯誘導(dǎo)組(EP組,n=30)、依托咪酯誘導(dǎo)/維持組(EE組,n=31)和丙泊酚對照組(PP組,n=30)。各組患者均依次給予咪達(dá)唑侖0.03mg·kg-1、芬太尼3~5μg·kg-1和順式阿曲庫銨1.5mg·kg-1靜脈注射后,EE組及EP組給予依托咪酯0.2~0.3mg·kg-1靜脈注射,PP組給予丙泊酚1.5~2.5mg·kg-1靜脈注射。術(shù)中EE組以0.6~1.2mg·kg-1·h-1的速度輸注依托咪酯;EP組及PP組以4~8mg·kg-1·h-1的速度輸注丙泊酚。于麻醉前、手術(shù)結(jié)束、術(shù)后24h抽取靜脈血,采用雙抗體夾心ELISA法檢測血清皮質(zhì)醇水平;于術(shù)前1d及術(shù)后1、7d使用簡易智能精神狀態(tài)檢查量表(MMSE)進(jìn)行評(píng)分,評(píng)定患者是否發(fā)生POCD。結(jié)果:各組術(shù)后1、7dPOCD發(fā)生率PP組為26.7%和13.3%,EP組為30%和10%,EE組為25.5%和12.9%,3組患者間術(shù)后1、7dPOCD發(fā)生率比較差異無統(tǒng)計(jì)學(xué)意義(P0.05)。3組患者術(shù)前皮質(zhì)醇濃度比較差異無統(tǒng)計(jì)學(xué)意義(P0.05),但3組患者術(shù)畢皮質(zhì)醇濃度均明顯低于術(shù)前水平(P0.05);與術(shù)前及EP組、PP組比較,EE組患者術(shù)畢皮質(zhì)醇濃度明顯降低(P0.05),術(shù)后24h3組患者皮質(zhì)醇濃度基本恢復(fù)至術(shù)前水平。結(jié)論:依托咪酯和丙泊酚均抑制老年骨科患者圍術(shù)期應(yīng)激反應(yīng),但依托咪酯麻醉對應(yīng)激反應(yīng)抑制程度強(qiáng)于丙泊酚;依托咪酯麻醉對老年骨科患者早期POCD的影響與丙泊酚相似,且POCD的發(fā)生與血清皮質(zhì)醇水平無關(guān)。
