【摘要】：目的探討局部浸潤(rùn)麻醉下小切口甲狀腺腺瘤切除術(shù)的方法、效果和可行性。方法選擇2014-04—2015-04接受局部浸潤(rùn)麻醉下小切口甲狀腺腺瘤切除術(shù)的36例患者為觀察對(duì)象。均經(jīng)術(shù)前彩超掃查、術(shù)中快速冰凍病理學(xué)檢查和術(shù)后常規(guī)病理學(xué)檢查確診�；仡櫺苑治龌颊叩呐R床資料。結(jié)果 36例患者均成功完成手術(shù),切口長(zhǎng)(4.05±0.36)cm,手術(shù)時(shí)間(75.31±13.76)min,術(shù)中出血量(14.31±5.22)m L。未發(fā)生喉返神經(jīng)損傷、繼發(fā)出血和甲狀旁腺功能低下等并發(fā)癥。術(shù)后未給予甲狀腺素治療,隨訪6~12個(gè)月,切口瘢痕細(xì)小且隱蔽在頸橫紋內(nèi)。甲狀腺功能正常。結(jié)論對(duì)經(jīng)彩超掃查并經(jīng)術(shù)中快速冰凍病理學(xué)檢查確診的單發(fā)甲狀腺腺瘤患者,局部浸潤(rùn)麻醉下實(shí)施小切口甲狀腺腺瘤切除術(shù),創(chuàng)傷小、并發(fā)癥少且切口瘢痕細(xì)小而隱蔽,療效肯定。
[Abstract]:Objective to explore the method, effect and feasibility of small incision thyroidectomy under local infiltration anesthesia. Methods Thirty-six patients undergoing small incision thyroidectomy under local infiltration anesthesia were enrolled in this study. All cases were diagnosed by preoperative color Doppler ultrasonography, intraoperative frozen pathological examination and postoperative routine pathological examination. The clinical data of the patients were analyzed retrospectively. Results the length of incision was (4.05 鹵0.36) cm, and the operative time was (75.31 鹵13.76) min,. The intraoperative bleeding volume was (14.31 鹵5.22) m L.. There were no complications such as recurrent laryngeal nerve injury, secondary hemorrhage and hypoparathyroidism. No thyroxine was given after operation. The incision scar was small and concealed in the transverse stria of neck after 6-12-month follow-up. Thyroid function is normal. Conclusion small incision thyroidectomy is performed under local infiltration anesthesia for patients with single thyroid adenoma diagnosed by color Doppler ultrasonography and rapid frozen pathological examination during operation. The results show that the small incision thyroidectomy is less invasive, less complication and smaller and concealed incision scar. The curative effect is definite.
