【摘要】：目的:研究右旋美托咪定(dexmedetomidine,Dex)對(duì)全麻甲狀腺手術(shù)患者接受纖維支氣管鏡(fiberoptic bronchoscope,FOB)引導(dǎo)經(jīng)口氣管插管的影響。方法:擬行經(jīng)口氣管插管全麻甲狀腺手術(shù)患者80例,美國(guó)麻醉醫(yī)師協(xié)會(huì)(American Society of Anesthesiology,ASA)I~Ⅲ級(jí),年齡29~71歲,隨機(jī)分為兩組(n=40):Dex組(D組)及安慰劑組(P組)。誘導(dǎo)前兩組患者分別以Dex(1μg·kg-1,4μg·m L-1)和等量生理鹽水泵入15 min,隨后常規(guī)麻醉誘導(dǎo),以FOB行經(jīng)口氣管插管,記錄入室后5 min(基礎(chǔ)值)、麻醉誘導(dǎo)前、氣管插管前、氣管插管即刻及氣管插管后1,2,3,5,10 min的血壓、心率和腦電雙頻指數(shù)(bispectral idex,BIS)。觀察患者氣管插管的血流動(dòng)力學(xué)變化及相關(guān)并發(fā)癥。結(jié)果:與P組患者相比,麻醉誘導(dǎo)和氣管插管期間,D組患者血流動(dòng)力學(xué)平穩(wěn)(P0.05),并發(fā)癥發(fā)生率低(但P0.05)。結(jié)論:麻醉誘導(dǎo)前輸注Dex,可以減輕FOB引導(dǎo)氣管插管血液動(dòng)力學(xué)反應(yīng)并減少其相關(guān)并發(fā)癥。
