【摘要】：目的:探討持續(xù)硬膜外麻醉應(yīng)用于臨產(chǎn)孕婦的分娩鎮(zhèn)痛效果及其對(duì)妊娠結(jié)局的影響,為更好的開(kāi)展硬膜外分娩鎮(zhèn)痛提供依據(jù)。方法:將600例產(chǎn)婦隨機(jī)分為鎮(zhèn)痛組和對(duì)照組,每組300例,鎮(zhèn)痛組實(shí)行持續(xù)性硬膜外分娩鎮(zhèn)痛,對(duì)照組常規(guī)分娩處理。觀察并記錄兩組鎮(zhèn)痛效果、產(chǎn)程時(shí)間、縮宮素使用情況、分娩方式、產(chǎn)婦不良反應(yīng)和新生兒情況。結(jié)果:鎮(zhèn)痛組鎮(zhèn)痛效果明顯優(yōu)于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);鎮(zhèn)痛組妊娠時(shí)活躍期、第二產(chǎn)程和總產(chǎn)程較對(duì)照組均有所延長(zhǎng),差異均有統(tǒng)計(jì)學(xué)意義(P0.05),第三產(chǎn)程時(shí)間比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);鎮(zhèn)痛組催產(chǎn)素使用率明顯高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05),鎮(zhèn)痛組尿潴留發(fā)生率較對(duì)照組高,差異有統(tǒng)計(jì)學(xué)意義(P0.05);兩組產(chǎn)婦產(chǎn)后出血情況比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),鎮(zhèn)痛組孕婦剖宮產(chǎn)率、新生兒窒息率和胎兒宮內(nèi)窘迫率均低于對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:持續(xù)硬膜外麻醉應(yīng)用于臨產(chǎn)孕婦的分娩鎮(zhèn)痛能減輕分娩疼痛,降低新生兒窒息率,改善孕婦的分娩結(jié)局,但有延長(zhǎng)產(chǎn)程,造成尿潴留的危險(xiǎn)。
[Abstract]:Aim: to investigate the effect of continuous epidural anesthesia on labor analgesia in parturient pregnant women and its effect on pregnancy outcome in order to provide evidence for better development of epidural analgesia in labor. Methods: 600 parturients were randomly divided into analgesic group and control group with 300 cases in each group. The analgesic group was given continuous epidural analgesia and the control group was treated with routine delivery. The analgesic effect, duration of labor, usage of oxytocin, mode of delivery, side effects of parturient and neonatal condition were observed and recorded. Results: the analgesic effect of the analgesia group was significantly better than that of the control group, the difference was statistically significant (P0.05). The active period of pregnancy, the second stage of labor and the total stage of labor in the analgesic group were longer than those in the control group, the difference was statistically significant (P0.05), and there was no significant difference in the third stage of labor time (P0.05); The utilization rate of oxytocin in the analgesic group was significantly higher than that in the control group (P0.05). The incidence of urinary retention in the analgesic group was higher than that in the control group (P0.05). There was no significant difference in postpartum hemorrhage between the two groups (P0.05). The cesarean section rate, neonatal asphyxia rate and fetal distress rate in the analgesia group were significantly lower than those in the control group (P0.05). Conclusion: continuous epidural anesthesia can relieve labor pain, reduce the rate of neonatal asphyxia and improve the outcome of labor, but it can prolong the labor process and lead to the risk of urine retention.
