異丙酚不同鎮(zhèn)靜深度對(duì)全麻老年患者腦氧代謝的影響

【摘要】:目的:評(píng)價(jià)異丙酚不同鎮(zhèn)靜深度對(duì)全麻老年患者腦氧代謝的影響。方法:選擇腹腔鏡下膽囊切除術(shù)老年患者60例,ASA分級(jí)Ⅰ或Ⅱ級(jí),年齡65~75歲,體質(zhì)量45~75 kg。采用隨機(jī)數(shù)字表法分為3組(n=20)。并根據(jù)異丙酚靶控濃度分為A組(3μg/m L)、B組(4μg/m L)、C組(5μg/m L),在麻醉誘導(dǎo)后調(diào)整各組靶控濃度,于麻醉誘導(dǎo)前(T0)、氣管插管時(shí)(T1)、切皮時(shí)(T2)、手術(shù)結(jié)束時(shí)(T3)時(shí)經(jīng)橈動(dòng)脈、頸內(nèi)靜脈球部采集血樣行血?dú)夥治?計(jì)算動(dòng)脈-頸內(nèi)靜脈血氧含量差(Da-jv O2)和腦氧攝取率(CERO2)。結(jié)果 :B、C兩組T2、T3時(shí)Da-jv O2和CERO2顯著低于A組,而Cjv O2則顯著高于A組;而B(niǎo)、C兩組間差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);T1~T3時(shí)A組NI值顯著高于B組,B組NI值顯著高于C組(P0.05)。結(jié)論:異丙酚靶控濃度4μg/m L更能改善全麻老年患者的腦氧代謝,減少腦氧攝取率。
