【摘要】：目的:對右美托咪定在婦科手術(shù)麻醉誘導(dǎo)期舒芬太尼鎮(zhèn)痛和鎮(zhèn)靜效果的影響進行研究分析。方法:隨機選取80例需行婦科手術(shù)的病例,分為對照組和觀察組各40例。對照組采用舒芬太尼和生理鹽水,觀察組采用舒芬太尼和右美托咪定(Dex),對兩組在用藥前(A1)、用藥后(A2)、最高清醒程度時(A3)和用舒芬太尼5min后(A4)的鎮(zhèn)痛與鎮(zhèn)靜效果進行分析。結(jié)果:在A2時觀察組NTI比對照組A2時低,兩組在A2、A4時NTI值都明顯低于同組的A1時,在NTI值降至最低時觀察組所用時間明顯短于對照組,兩組A4時的PTO和PPT都明顯高于同組A1時,但觀察組在A1和A4時PTO和PPT差值明顯高于對照組,在惡心、發(fā)冷和嘔吐等不良反應(yīng)上觀察組同樣低于對照組,差異具有統(tǒng)計學(xué)意義(P0.05)。結(jié)論:Dex對婦科手術(shù)麻醉誘導(dǎo)期舒芬太尼鎮(zhèn)痛和鎮(zhèn)靜都有很好的作用,還能使鎮(zhèn)靜作用更為快速,不良反應(yīng)也較單獨用舒芬太尼少,在使用時還是應(yīng)注意對不良反應(yīng)的干預(yù)。
[Abstract]:Aim: to study the effect of dexmetomidine on analgesia and sedation of sufentanil during the induction of gynecological anesthesia. Methods: 80 cases of gynecologic operation were randomly divided into control group and observation group. The control group was treated with sufentanil and normal saline, the observation group was treated with sufentanil and dexmetomidine (Dex), before (A1) and after administration (A2). The analgesic and sedative effects were analyzed at maximum waking level (A 3) and after sufentanil 5min (A 4). Results: the NTI of the observation group was lower than that of the control group at A2, the NTI value of both groups was significantly lower than that of the control group at A2A4, and the time of the observation group at the lowest NTI value was significantly shorter than that in the control group. PTO and PPT in A4 group were significantly higher than those in A4 group, but the difference of PTO and PPT in A4 group was significantly higher than that in control group, and in adverse reactions such as nausea, chilling and vomiting, the difference of PTO and PPT in observation group was also lower than that in control group. The difference was statistically significant (P0.05). Conclusion: Dex has a good effect on the analgesia and sedation of sufentanil during the induction period of gynecological anesthesia, and can also make the sedation effect more rapid, and the adverse reaction is less than that of sufentanil alone, so we should pay attention to the intervention of the adverse reaction in the use of sufentanil.
