【摘要】：目的比較肥胖患者在超聲引導(dǎo)下神經(jīng)阻滯麻醉及盲探法神經(jīng)阻滯麻醉在麻醉穿刺時(shí)間、神經(jīng)阻滯效果、阻滯維持時(shí)間、穿刺并發(fā)癥及術(shù)后患者滿意度等方面的差別。方法選擇擬行上肢手術(shù)的肥胖患者40例,BMI30kg/m2,ASAⅠ~Ⅱ級。分為兩組,C組(盲探)和U組(超聲引導(dǎo)),每組20例。C組用傳統(tǒng)盲探法進(jìn)行神經(jīng)阻滯麻醉,U組在超聲引導(dǎo)下進(jìn)行神經(jīng)阻滯麻醉。術(shù)中記錄操作、感覺神經(jīng)阻滯起效、運(yùn)動(dòng)神經(jīng)阻滯起效、感覺神經(jīng)維持時(shí)間,術(shù)畢評定神經(jīng)阻滯效果,記錄不良反應(yīng)的發(fā)生情況及術(shù)后患者的滿意度。結(jié)果 U組的麻醉操作時(shí)間(9.1±1.8)min明顯長于C組(6.9±1.9)min;與C組比較,U組神經(jīng)阻滯起效時(shí)間明顯縮短且U組維持時(shí)間明顯延長(P0.01);U組術(shù)后患者的滿意度明顯高于C組(P0.01)。結(jié)論超聲引導(dǎo)下神經(jīng)阻滯對于肥胖患者是一種安全有效、并發(fā)癥少的新型技術(shù),能夠明顯提高患者的術(shù)后滿意度,為患者提供更加舒適化的服務(wù)。
