【摘要】：目的系統(tǒng)評價加巴噴丁術前用藥對開腹子宮切除術患者術前用藥的效果及安全性。方法電子檢索PubMed、Cochrane、Embase、Ovid、Springer Link、Web of Science、CNKI、萬方、維普等數據庫,收集2000~2016年發(fā)表的加巴噴丁術前用藥的隨機對照試驗(randomized controlled trial,RCT)文獻,按Cochrane指導手冊的方法選擇文獻、提取資料及評價研究質量后,采用RevMan5.3軟件進行Meta分析。結果共納入10篇RCT文獻,共計609例患者。Meta分析結果顯示,與對照組比較,加巴噴丁組術后24h阿片類麻醉藥物的累積用量明顯減少(WMD=-8.83mg,95%CI-12.70~-4.97,P0.001);VAS評分無明顯降低(WMD=-7.70mm,95%CI-18.22~2.82,P=0.15);嘔吐的發(fā)生率明顯降低(RR=0.49,95%CI 0.35~0.69,P0.001)。兩組術后24h內惡心的發(fā)生率(RR=0.73,95%CI 0.45~1.17,P=0.19)和嗜睡的發(fā)生率(RR=2.08,95%CI0.62~6.92,P=0.23)差異無統(tǒng)計學意義。結論開腹子宮切除術的患者加巴噴丁術前用藥能夠有效地降低阿片類麻醉藥物的累積用量,減少嘔吐的發(fā)生率,而對術后疼痛、惡心以及嗜睡無明顯影響。
[Abstract]:Objective to evaluate the efficacy and safety of gabapentin before operation in patients undergoing open hysterectomy. Methods PubMed,Cochrane,Embase,Ovid,Springer Link,Web of Science,CNKI, Wanfang, Wiper and other databases were searched electronically, and the literature on the randomized controlled trial (randomized controlled trial,RCT) of gabapentin before operation was collected from 2000 to 2016. According to the method of Cochrane guidebook, the documents were selected, the data were extracted and the research quality was evaluated. The Meta analysis was carried out with RevMan5.3 software. Results A total of 609 patients were included in 10 articles of RCT. The results of Meta analysis showed that compared with the control group, the cumulative dosage of opioid anesthetic in gabapentin group decreased significantly 24 hours after operation (WMD=-8.83mg,95%CI-12.70~-4.97,). (P0.001); There was no significant decrease in VAS score (WMD=-7.70mm,95%CI-18.22~2.82,P=0.15) and a significant decrease in the incidence of vomiting (RR=0.49,95%CI 0.35 0. 69 鹵0. 001). There was no significant difference in the incidence of nausea (RR=0.73,95%CI 0.45) and somnolence (RR=2.08,95%CI0.62~6.92,P=0.23) between the two groups within 24 hours after operation. Conclusion preoperative administration of gabapentine in patients undergoing open hysterectomy can effectively reduce the cumulative dosage of opioid anesthetic and reduce the incidence of vomiting, but has no effect on postoperative pain, nausea and drowsiness.
