【摘要】：目的探討簡便氣道管理流程在全身麻醉快誘導(dǎo)氣管插管中的應(yīng)用價(jià)值。 方法回顧分析2005年1月～2012年12月9251例全身麻醉快誘導(dǎo)氣管插管病歷資料。根據(jù)使用簡便氣道管理流程的時(shí)間將數(shù)據(jù)分為兩組，，2009年1月～2012年12月使用簡便氣道流程期間的病歷為A組（n=4214），2005年1月～2008年12月未使用簡便氣道流程期間的病歷為B組（n=5037）。分析A組氣道管理方案的構(gòu)成比，用Kappa檢驗(yàn)分析初步方案的應(yīng)用效果，用χ2檢驗(yàn)比較A組與B組聲門顯露Ⅲ～Ⅳ級(jí)氣管插管方法的構(gòu)成比。 結(jié)果1.A組4214例病歷全部氣管插管成功，未發(fā)生因氣管插管導(dǎo)致的嚴(yán)重缺氧、腦損傷及心搏驟停。麻醉誘導(dǎo)期10.87%（458/4214）出現(xiàn)了氣道問題。A組有3.70%（156/4214）出現(xiàn)了面罩通氣問題：雙手扣面罩通氣占8.98%（14/156）、口咽通氣道輔助面罩通氣占89.74%（140/156）、使用喉罩通氣占1.28%（2/156）。A組有7.17%（302/4214）出現(xiàn)了氣管插管問題：經(jīng)口盲探法氣管插管占33.44%（102/302）、彈性橡膠探條輔助Macintosh喉鏡氣管插管占62.26%（188/302）、McCoy喉鏡氣管插管占3.97%（12/302）、纖維支氣管鏡氣管插管占0.33%（1/302）。2.A組出現(xiàn)面罩通氣問題后首選口咽通氣道的Kappa值=0.95，出現(xiàn)氣管插管問題后首選彈性橡膠探條的Kappa值=0.73。3.聲門顯露Ⅲ～Ⅳ級(jí)A組6.12%（258/4214），B組6.83%（344/5037），兩組間比較無統(tǒng)計(jì)學(xué)差異(P＞0.05)。兩組聲門顯露Ⅲ～Ⅳ級(jí)氣管插管的方法比較有統(tǒng)計(jì)學(xué)差異(P＜0.01)： A組使用彈性橡膠探條輔助Macintosh喉鏡氣管插管的使用率為56.20%（145/258）、B組為40.40%（139/344）、兩組間比較有統(tǒng)計(jì)學(xué)差異(P＜0.05)；A組經(jīng)口盲探法39.15%（101/258）、B組50.58%（174/344）、兩組間比較有統(tǒng)計(jì)學(xué)差異(P＜0.05)；A組其它方法的使用率4.65%(12/258)、B組9.01%（31/344）、兩組間比較有統(tǒng)計(jì)學(xué)差異(P＜0.05)。 結(jié)論本簡便氣道管理流程具有一定的臨床應(yīng)用價(jià)值。
[Abstract]:Objective to investigate the application value of simple airway management procedure in fast induction tracheal intubation under general anesthesia. Methods from January 2005 to December 2012, 9251 cases of rapid induction tracheal intubation under general anesthesia were retrospectively analyzed. The data were divided into two groups according to the time of using the simple airway management process. The medical records for the period from January 2009 to December 2012 were group A (n = 4214). The medical records of group B during the period from January 2005 to December 2008 without the use of simple airway procedures were group B (n = 5037). The composition ratio of airway management scheme in group A was analyzed, the application effect of the preliminary scheme was analyzed by Kappa test, and the composition ratio of glottis exposure grade 鈪
