【摘要】：目的探討COX-2特異性抑制劑帕瑞昔布對(duì)老年膝關(guān)節(jié)置換患者術(shù)后血清炎癥因子及早期認(rèn)知功能的影響。方法選取72例擇期行膝關(guān)節(jié)置換老年患者,分為生理鹽水組(Ⅰ組)與帕瑞昔布組(Ⅱ組)。比較兩組術(shù)前1d及術(shù)后1d、3d、7d簡(jiǎn)明精神狀態(tài)檢查量表(MMSE)評(píng)分。在麻醉誘導(dǎo)前(T1)、術(shù)后6h(T2)、術(shù)后1d(T3)、術(shù)后3d(T4)、7d(T5)行血清IL-1β、IL-6濃度測(cè)定,T2時(shí)測(cè)定血S-100β水平,兩組間進(jìn)行比較。結(jié)果術(shù)后1d、3dⅠ組MMSE評(píng)分較術(shù)前降低明顯(P0.05)。Ⅰ組IL-6與IL-1β于T2開(kāi)始升高,T5降至術(shù)前水平,而Ⅱ組T3開(kāi)始升高,T4降至術(shù)前水平。Ⅰ組IL-6在T2、T3、T4時(shí),IL-1β在T3、T4均高于Ⅱ組(P0.05)。Ⅰ組T2S-100β為(1 625±364)pg/mL,Ⅱ組為(1 263±174)pg/mL,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論COX-2特異性抑制劑帕瑞昔布對(duì)老年骨科手術(shù)患者術(shù)后認(rèn)知功能的下降具有保護(hù)作用,可能與其抑制外周細(xì)胞因子釋放有關(guān)。
