【摘要】：目的探討超聲引導(dǎo)下神經(jīng)阻滯復(fù)合喉罩全麻在脛骨骨折手術(shù)中的應(yīng)用效果。方法研究我院在2014年5月至2015年11月期間收治的80例脛骨骨折手術(shù)患者,分為對照組和觀察組各40例,其中對照組運用喉罩全麻,觀察組運用超聲引導(dǎo)下神經(jīng)阻滯復(fù)合喉罩全麻,分析兩組患者麻醉相關(guān)指標(biāo)差異。結(jié)果拔管后半小時VAS評分上,觀察組(1.8±0.3)分顯著低于對照組(4.0±0.4)分,兩組差異具有統(tǒng)計學(xué)意義,P0.05;在麻醉后的動脈壓指數(shù)上,對照組均低于對照組,兩組差異具有統(tǒng)計學(xué)意義,P0.05;在喚醒時間和拔管時間上,觀察組各項均早于對照組,兩組差異具有統(tǒng)計學(xué)意義,P0.05;在術(shù)后并發(fā)癥發(fā)生率上,對照組為2.5%,觀察組為15%,兩組差異具有統(tǒng)計學(xué)意義,P0.05。結(jié)論超聲引導(dǎo)下神經(jīng)阻滯復(fù)合喉罩全麻在脛骨骨折手術(shù)中運用可以有效保持生命體征穩(wěn)定,麻醉鎮(zhèn)痛效果佳,術(shù)后蘇醒恢復(fù)快,麻醉效果與安全性高。
[Abstract]:Objective to investigate the effect of ultrasound-guided nerve block combined with laryngeal mask general anesthesia in the operation of tibial fracture. Methods from May 2014 to November 2015, 80 patients with tibial fractures treated in our hospital were divided into control group (n = 40) and observation group (n = 40). The patients in the observation group were treated with nerve block combined with laryngeal mask anesthesia under the guidance of ultrasound to analyze the difference of anaesthesia related indexes between the two groups. Results the VAS score of the observation group (1.8 鹵0.3) was significantly lower than that of the control group (4.0 鹵0.4) at half hour after extubation, and the difference between the two groups was statistically significant (P 0.05). The arterial pressure index after anesthesia in the control group was lower than that in the control group, and the difference between the two groups was statistically significant (P 0.05). The arousal time and extubation time in the observation group were earlier than those in the control group, the difference between the two groups was statistically significant (P 0.05). The incidence of postoperative complications in the control group was 2.5 and that in the observation group was 15. The difference between the two groups was statistically significant (P 0.05). Conclusion Ultrasound-guided nerve block combined with laryngeal mask general anesthesia can effectively maintain the stability of vital signs and analgesia, recover quickly after operation, and have high anesthetic effect and safety.
