【摘要】：目的:比較針刺麻醉與西藥局部阻滯麻醉行拔牙術(shù)的臨床療效差異,探尋臨床拔牙術(shù)的較佳麻醉方法。方法:將60例拔牙患者隨機(jī)分為針麻組和藥麻組,每組30例。針麻組針刺合谷、內(nèi)庭,患側(cè)下關(guān)、頰車等,得氣后行拔牙術(shù),留針至術(shù)后10 min;藥麻組選用復(fù)方鹽酸阿替卡因注射液常規(guī)局部麻醉后行拔牙術(shù)。對(duì)兩組麻醉效果及鎮(zhèn)痛效果[疼痛分級(jí)指數(shù)(PRI)評(píng)分及視覺模擬評(píng)分(VAS)]進(jìn)行臨床對(duì)比觀察,并觀察患者術(shù)中出血量、不良反應(yīng)發(fā)生率及手術(shù)前后血壓、脈搏、呼吸情況。結(jié)果:兩種麻醉方法下,兩組患者均成功完成拔牙術(shù)。兩組麻醉評(píng)級(jí)優(yōu)良率、PRI評(píng)分、VAS評(píng)分比較差異均無統(tǒng)計(jì)學(xué)意義(均P0.05);針麻組術(shù)中出血量少于藥麻組(P0.01),不良反應(yīng)發(fā)生率低于藥麻組(P0.05);手術(shù)前后兩組患者血壓、脈搏、呼吸都在正常范圍內(nèi),兩組間比較差異無統(tǒng)計(jì)學(xué)意義(均P0.05)。結(jié)論:針刺麻醉下行拔牙術(shù)可達(dá)到西藥局部阻滯的麻醉效果,鎮(zhèn)痛完全,且術(shù)中生命體征平穩(wěn)、不良反應(yīng)少,可作為臨床拔牙術(shù)的麻醉方法之一。
[Abstract]:Objective: to compare the clinical curative effect between acupuncture anesthesia and local block anesthesia. Methods: 60 patients with tooth extraction were randomly divided into acupuncture anesthesia group and drug anesthesia group with 30 cases in each group. In the acupuncture group, Hegu, chamber, affected side Xiaguan, buccal car and so on were treated with extraction of teeth after getting qi, and the group of drug anesthesia with compound Atevacaine hydrochloride was used after routine local anesthesia for 10 min; after the operation. The anaesthesia effect and analgesic effect [pain grading index (PRI) score and visual analog score (VAS)] were compared between the two groups. The amount of intraoperative bleeding, the incidence of adverse reactions, blood pressure and pulse before and after operation were observed. Breathing. Results: under the two anesthesia methods, the two groups of patients successfully completed tooth extraction. There was no significant difference in the excellent and good rate of anesthesia rating, PRI score and VAS score between the two groups (P0.05); the amount of intraoperative bleeding in the acupuncture anesthesia group was less than that in the drug anesthesia group (P0.01), and the incidence of adverse reactions was lower than that in the drug anesthesia group (P0.05). Before and after operation, the blood pressure, pulse, respiration were in the normal range, there was no significant difference between the two groups (P0.05). Conclusion: extraction of teeth under acupuncture anesthesia can achieve the anesthetic effect of local block of western medicine, complete analgesia, stable vital signs and less adverse reactions during the operation, so it can be used as one of the anesthetic methods for clinical extraction of teeth.
