【摘要】：目的回顧性分析局部麻醉下應(yīng)用李金斯坦(Lichtenstein)平片改良修補(bǔ)腹股溝疝的臨床效果。方法以2%鹽酸利多卡因15 ml+2.5%鹽酸布比卡因15 ml+0.9%生理鹽水30 ml+腎上腺素0.3 mg混合總量約60 ml,手術(shù)切口標(biāo)記線上每距離1~1.5 cm處作分點(diǎn)局部浸潤(rùn)麻醉。麻醉達(dá)成后處理好疝囊,再將李金斯坦平片置入加強(qiáng)精索后壁或增強(qiáng)股管張力,并觀察本院經(jīng)李金斯坦平片改良修補(bǔ)腹股溝疝和其他手術(shù)方式Rutkow、Kugel、M-Kugel、PHS、TEP在住院時(shí)間、費(fèi)用、術(shù)后復(fù)發(fā)、術(shù)后慢性疼痛等指標(biāo)方面的變化。結(jié)果從本院在腹股溝疝修補(bǔ)術(shù)中的手術(shù)方式對(duì)比中,局麻下李金斯坦平片改良修補(bǔ)術(shù)后住院時(shí)間和費(fèi)用明顯低于其他手術(shù)方式(P0.05),術(shù)后復(fù)發(fā)及慢性疼痛均較低,無(wú)顯著差異。結(jié)論局麻下李金斯坦平片改良修補(bǔ)術(shù)式能同時(shí)覆蓋腹股溝區(qū)所有疝突出的部位,且住院時(shí)間短、費(fèi)用較低、術(shù)后疼痛輕、復(fù)發(fā)概率低等優(yōu)點(diǎn),在基礎(chǔ)醫(yī)院即可實(shí)施,值得臨床推廣。
[Abstract]:Objective to retrospectively analyze the clinical effect of modified (Lichtenstein) film in repairing inguinal hernia under local anesthesia. Methods Lidocaine hydrochloride was used for 15 ml 2.5% bupivacaine hydrochloride 15 ml 0.9% normal saline 30 ml epinephrine 0.3 mg mixed total amount about 60 ml, surgical incision labeling line at 1: 1. 5 cm for local infiltration anesthesia. After anaesthesia was achieved, the herniation sac was treated well, and then the Li Jin Stapine film was placed into the posterior wall of spermatic cord or increased the tension of the femoral tube, and the hospital treated inguinal hernia and other surgical methods Rutkow,Kugel,M-Kugel,PHS,TEP with the modified Li Jin Steppin film were observed to be in hospital for the duration, cost, and recurrence after operation. Changes of chronic pain and other indexes after operation. Results according to the comparison of surgical methods during inguinal hernioplasty, the hospitalization time and cost of modified repair of Li Jin Steppin film under local anesthesia were significantly lower than those of other surgical methods (P0.05), and the recurrence and chronic pain were lower than those of other surgical methods (P0.05), and there was no significant difference in the postoperative recurrence and chronic pain. Conclusion under local anesthesia, the modified repair of Li Jin Steppin tablet can cover all hernia protruding sites in the inguinal area at the same time, and the hospital stay is short, the cost is low, the postoperative pain is light, the recurrence probability is low, and so on, which can be carried out in the basic hospital. It is worth popularizing in clinic.
