【摘要】：目的建立經(jīng)胃內(nèi)鏡及腹腔鏡腹腔探查的雄兔模型,對(duì)比兩者對(duì)急性炎性反應(yīng)的影響。方法雄性日本大耳白兔隨機(jī)分為經(jīng)胃內(nèi)鏡(N組,n=8)及腹腔鏡(L組,n=8)腹腔探查組,假手術(shù)組(S組)5只,酶聯(lián)免疫吸附試驗(yàn)(ELISA)檢測術(shù)前、術(shù)后4、24及72 h血清腫瘤壞死因子-α(TNF-α)、白細(xì)胞介素(IL)-6及IL-10的水平。結(jié)果動(dòng)物模型建立成功,N組手術(shù)時(shí)間較L組長,前者出現(xiàn)胃造口漏1例;TNF-α、IL-6及IL-10水平在術(shù)前各組間及S組麻醉前后差異無統(tǒng)計(jì)學(xué)意義;N、L組TNF-α術(shù)后4 h明顯上升,N組上升滯后,但維持時(shí)間長,術(shù)后4 h L組高于N組,術(shù)后24 h相反;N、L組IL-6術(shù)后24 h明顯上升,N組術(shù)后72 h下降,L組不明顯,術(shù)后4 h L組低于N組,術(shù)后72 h相反;N組術(shù)后IL-10較術(shù)前變化不明顯,L組術(shù)后24 h明顯下降,持續(xù)至72h,術(shù)后24及72 h N組均高于L組。結(jié)論經(jīng)胃內(nèi)鏡及腹腔鏡腹腔探查的雄兔模型是可行的,前者耗時(shí)較長,胃造口漏是嚴(yán)重的并發(fā)癥;兩者均引發(fā)全身促炎反應(yīng),但出現(xiàn)及維持時(shí)間存在差異,前者抗炎反應(yīng)變化不明顯,后者術(shù)后24及72 h持續(xù)低下。
[Abstract]:Objective to establish a male model of intraperitoneal exploration by endoscopy and laparoscopy, and to compare the effects of the two models on acute inflammatory reaction. Methods male Japanese large ear white rabbits were randomly divided into two groups: intraperitoneal exploration group (group N) and laparoscopy (group L), and sham operation group (group S). (ELISA) was detected by Elisa before operation. Serum levels of tumor necrosis factor- 偽 (TNF- 偽), interleukin (IL) 6 and IL-10 at 24 and 72 hours after operation. Results the operation time of group N was longer than that of group L, the level of TNF- 偽 IL-6 and IL-10 in 1 case with gastric leakage was not significantly different between each group before operation and before and after anesthesia in group S. The increase of TNF- 偽 in group N was significantly higher than that in group L 4 h after operation. However, the maintenance time was longer, 4 h after operation in L group was higher than that in N group, on the contrary, 24 h after operation, the IL-6 in group N increased significantly, and the decrease of IL-6 in group N at 72 h after operation was not obvious, but that in group L was lower than that in group N at 4 h after operation. On the contrary, the IL-10 of group N was significantly lower than that of group L at 24 h after operation, and lasted for 72 h, and that of group N was higher than that of group L at 24 h and 72 h postoperatively. Conclusion it is feasible to explore the male rabbit model by gastroendoscopy and laparoscopy, the former takes a long time and the gastric leakage is a serious complication, both of them cause systemic inflammatory reaction, but there are differences in the time of appearance and maintenance. The former had no obvious change in anti-inflammatory reaction, while the latter had a low level at 24 and 72 hours after operation.
