【摘要】：目的探討食管癌患者圍手術(shù)期抗菌藥物的選用與術(shù)后肺部感染的相關(guān)性,為臨床合理用藥提供參考。方法收集2012年4-11月醫(yī)院284例食管癌手術(shù)患者臨床資料,包括手術(shù)方式、美國麻醉師協(xié)會(huì)評(píng)分等數(shù)據(jù),詳細(xì)記錄手術(shù)預(yù)防用抗菌藥物的名稱、劑量、初始給藥及追加時(shí)間、手術(shù)持續(xù)時(shí)間等,使用SPSS 21.0統(tǒng)計(jì)軟件進(jìn)行單變量及多變量分析。結(jié)果 284例患者術(shù)后發(fā)生肺部感染68例,感染率23.94%;肺部感染患者痰標(biāo)本分離出革蘭陰性菌29株占96.67%,革蘭陽性菌1株占3.33%;術(shù)前給藥時(shí)機(jī)正確的有258例,占90.84%;210例患者手術(shù)持續(xù)時(shí)間3h,僅146例追加使用了抗菌藥物;68例肺部感染患者使用第一代頭孢菌素,患者使用第一、二代頭孢菌素或頭孢曲松及β-內(nèi)酰胺酶抑制劑復(fù)方制劑后,肺部感染發(fā)生率分別為27.23%和17.20%,術(shù)前給藥時(shí)機(jī)對(duì)術(shù)后肺部感染發(fā)生有顯著影響,術(shù)中追加與否對(duì)術(shù)后肺部感染發(fā)生無顯著影響。結(jié)論食管癌患者圍手術(shù)期抗菌藥物的選擇對(duì)術(shù)后肺部感染并無影響,應(yīng)嚴(yán)格遵循相關(guān)指南推薦用藥。
[Abstract]:Objective to investigate the relationship between the choice of antibiotics and postoperative pulmonary infection in patients with esophageal cancer. Methods the clinical data of 284 patients with esophageal cancer from April to November 2012 were collected, including the operation method, the score of American anaesthetists and so on. The names, doses, initial administration and supplementary time of antibiotics for surgical prophylaxis were recorded in detail. The operation duration was analyzed by using SPSS 21. 0 statistical software for univariate and multivariate analysis. Results Sixty-eight cases of postoperative pulmonary infection occurred in 284 patients, and the infection rate was 23.940.The sputum samples of the patients with pulmonary infection isolated 29 Gram-negative bacteria (96.67m) and one Gram-positive bacteria (3.33). The operation lasted for 3 hours in 210 patients. Only 146 patients received additional antibiotics and 68 patients with pulmonary infection received the first generation cephalosporin, the first and second generation cephalosporins or ceftriaxone and 尾 -lactamases inhibitor compound preparation. The incidence of pulmonary infection was 27.23% and 17.20%, respectively. The timing of preoperative administration had a significant effect on the incidence of postoperative pulmonary infection, but there was no significant effect of intraoperative supplementation on postoperative pulmonary infection. Conclusion the choice of antibiotics in patients with esophageal cancer during perioperative period has no effect on postoperative pulmonary infection.
