【摘要】：目的探討研究Narcotrend監(jiān)測(cè)儀在老年患者全身麻醉中麻醉深度的臨床應(yīng)用。方法選擇我院2013年2月-2014年12月期間我院全身麻醉下行直腸癌手術(shù)的老年患者90例,隨機(jī)分成淺麻醉組、深麻醉組和對(duì)照組,各30例。淺麻醉組和深麻醉組使用常規(guī)監(jiān)測(cè)外加Narcotrend監(jiān)測(cè)儀監(jiān)測(cè)患者的麻醉深度。觀察比較3組患者的手術(shù)時(shí)間(t1)、術(shù)后睜眼時(shí)間(t2)、拔管時(shí)間(t3)、術(shù)后定向力的恢復(fù)時(shí)間(t4)以及瑞芬太尼和丙泊酚的用量;調(diào)查患者術(shù)中知曉情況和術(shù)后認(rèn)知功能障礙(POCD)的發(fā)生情況。結(jié)果 3組患者的t2、t3、t4以及丙泊酚的用量依次升高且兩兩之間的差異均有統(tǒng)計(jì)學(xué)意義(P0.05),3組患者t1和瑞芬太尼的用量之間差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。手術(shù)中3組患者均未出現(xiàn)術(shù)中知曉的情況,深麻醉組有1名患者(3.33%)出現(xiàn)POCD,其他兩組沒有出現(xiàn)。結(jié)論在老年患者全身麻醉中應(yīng)用Narcotrend監(jiān)測(cè)儀監(jiān)測(cè)麻醉深度可以適當(dāng)減少麻醉藥用量,縮短患者蘇醒時(shí)間、促進(jìn)患者恢復(fù),具有重要的臨床指導(dǎo)意義。
[Abstract]:Objective to investigate the clinical application of Narcotrend monitor in general anesthesia of elderly patients. Methods from February 2013 to December 2014, 90 elderly patients undergoing rectal cancer surgery under general anesthesia in our hospital were randomly divided into three groups: superficial anesthesia group, deep anesthesia group and control group (30 cases each). Routine monitoring and Narcotrend monitor were used to monitor the anesthetic depth in the shallow anesthesia group and the deep anesthesia group. The operative time (T1), postoperative eye opening time (T2), extubation time (T3), recovery time of postoperative orientation (t4) and the dosage of remifentanil and propofol were observed and compared among the three groups. Results the dosage of t2t3, t4 and propofol increased in turn and the difference was significant (P0.05). There was no significant difference between the dosage of T1 and remifentanil in the three groups (P0.05). There was no intraoperative knowledge in all the three groups, and one patient (3.33%) in the deep anesthesia group had no POCD, in the other two groups. Conclusion the application of Narcotrend monitor to monitor the depth of anesthesia in elderly patients with general anesthesia can reduce the dosage of anesthetic, shorten the time of recovery and promote the recovery of patients. It has important clinical significance.
