【摘要】：目的觀察應(yīng)用右美托咪啶預(yù)防兒童七氟醚麻醉蘇醒期躁動的效果。方法將56例接受七氟醚麻醉手術(shù)的患兒隨機(jī)分為2組,每組28例。觀察組:麻醉誘導(dǎo)后靜脈泵注右美托咪定注射液0.5μg/kg(稀釋到15 m L),10~20 min注完后開始手術(shù)。對照組:麻醉誘導(dǎo)后手術(shù)開始前泵注15 m L生理鹽水。比較2組患兒右美托咪啶或生理鹽水注射前(T0)、注射5 min后(T1)、患兒蘇醒睜眼時(shí)間點(diǎn)(T2)及拔管5 min后(T3)心率、血壓變化和麻醉后的蘇醒時(shí)間、躁動情況等。結(jié)果對照組T2和T3時(shí)點(diǎn)的NAP和HR較T0明顯升高,差異有統(tǒng)計(jì)學(xué)意義(P0.05);與對照組比較,觀察組T2和T3時(shí)間點(diǎn)MAP明顯降低,同時(shí)T1、T2和T3時(shí)間點(diǎn)HR明顯降低,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組患兒蘇醒期躁動發(fā)生率低于對照組,蘇醒時(shí)間長于于對照組,2組比較差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論麻醉誘導(dǎo)后手術(shù)開始前應(yīng)用右美托咪啶,在兒童七氟醚麻醉時(shí)能較好維持血流動力學(xué)的平穩(wěn),降低蘇醒期的躁動發(fā)生率。
[Abstract]:Objective to observe the effect of dexmetidine on prevention of restlessness during sevoflurane anesthesia in children. Methods 56 children undergoing sevoflurane anesthesia were randomly divided into 2 groups with 28 cases in each group. Observation group: after anesthesia induction, intravenous injection of dexmetomidine 0.5 渭 g / kg (diluted to 15 mL / kg 1020 min) was performed. Control group: 15 mL saline was injected by pump before operation after anesthesia induction. The changes of heart rate, blood pressure, recovery time after anesthesia and restlessness were compared between the two groups before dexmetidine or normal saline injection, 5 min after injection, after 5 min, the recovery time was T2), and 5 min after extubation, the heart rate, blood pressure, recovery time after anesthesia, restlessness, and so on were compared between the two groups. Results compared with the control group, the map at T2 and T3 time points in the control group was significantly lower than that in the control group (P 0.05), and the HR at T 1 T 2 and T 3 time points was significantly lower than that in the control group (P 0.05). The incidence of restlessness in the observation group was lower than that in the control group, and the recovery time was longer than that in the control group (P 0.05). Conclusion before anesthesia induction, dexmetidine can maintain hemodynamic stability and reduce restlessness during sevoflurane anesthesia in children.
