超聲引導(dǎo)和傳統(tǒng)解剖定位法用于肌間溝臂叢神經(jīng)阻滯的麻醉效果比較

【摘要】:目的比較超聲引導(dǎo)和傳統(tǒng)解剖定位法用于肌間溝臂叢神經(jīng)阻滯的麻醉效果和安全性。方法 60例擇期行上肢手術(shù)患者,隨機均分成A、B兩組,A組在超聲引導(dǎo)下行肌間溝臂叢神經(jīng)阻滯,B組傳統(tǒng)解剖定位下行肌間溝臂叢神經(jīng)阻滯,局麻醉藥均使用2%利多卡因和0.75%左布比卡因混合液(1∶1)20mL。記錄比較兩組患者麻醉起效時間、維持時間、尺神經(jīng)阻滯完善情況、麻醉效果以及不良反應(yīng)。結(jié)果 A組患者麻醉起效時間短于B組,麻醉維持時間長于B組;A組患者麻醉優(yōu)良率明顯高于B組,無麻醉并發(fā)癥,而且尺神經(jīng)阻滯較完全。結(jié)論超聲引導(dǎo)肌間溝臂叢神經(jīng)阻滯起效快,維持時間長,麻醉效果佳,并發(fā)癥少,操作成功率高,尺神經(jīng)阻滯更完全,值得臨床推廣。
[Abstract]:Objective to compare the anesthetic effect and safety of ultrasound guided and traditional anatomical localization for intermuscular sulcus brachial plexus block. Methods Sixty patients undergoing elective upper limb operation were randomly divided into two groups: group A and group A were treated with ultrasound guided intermuscular sulcus brachial plexus block and group B were subjected to intermuscular sulcus brachial plexus block under traditional anatomic orientation. All the anesthetics were treated with 2% lidocaine and 0.75% levobupivacaine. The onset time, maintenance time, improvement of ulnar nerve block, anaesthesia effect and adverse reaction were recorded and compared between the two groups. Results the onset time of anesthesia in group A was shorter than that in group B, and the maintenance time of anesthesia in group A was longer than that in group B. The excellent and good rate of anesthesia in group A was significantly higher than that in group B. there were no complications of anesthesia and the ulnar nerve block was more complete. Conclusion Ultrasound-guided intermuscular sulcus brachial plexus block has the advantages of quick onset, long maintenance time, good anesthetic effect, less complications, high operational success rate, and more complete ulnar nerve block, which is worth popularizing in clinic.
