【摘要】：目的：觀察分析針刺百會、風府、神門、太沖穴對腦卒中后抑郁（Post-strokedepression,PSD）模型大鼠行為學、神經(jīng)功能缺損以及體重的影響，為針灸治療腦卒中后抑郁提供科學依據(jù)。 方法：采用大腦中動脈線拴阻塞法模型（MCAO），大鼠在清醒后第7天，相繼給予慢性溫和不可預測應激（CUMS）處理，，制備腦卒中伴抑郁的復合模型。運用Longa評分方法，進行5分制評分，以評定腦卒中大鼠神經(jīng)功能的缺損與恢復；運用糖水消耗實驗評定PSD大鼠興趣及快感的變化；運用敞箱實驗評定PSD大鼠對事物的好奇程度以及自發(fā)運動的變化等抑郁行為的改變。 結果：在MACO模型建立2小時大鼠清醒后進行神經(jīng)功能缺損評分，各組造模大鼠的Longa評分均在1-3分，24小時后大鼠神經(jīng)功能缺損基本恢復正常，Longa評分均為0分，說明大鼠在手術麻醉清醒后出現(xiàn)了病灶對側肢體輕度功能障礙，出現(xiàn)明顯神經(jīng)功能缺損，空白組與模型組大鼠體重以及行為學各項評估結果進行對比，模型組大鼠體重及行為學評估結果均有明顯降低（P＜0.05），提示PSD模型成功。治療后針刺組與西藥組大鼠體重及行為學評估結果顯示，兩治療組均療效顯著（P＜0.05），但兩組差異不明顯（P＞0.05）。 結論：采用大腦中動脈線拴阻塞法模型復合慢性不可預知性應激抑郁癥模型可以成功制備腦卒中后抑郁的動物模型。針刺百會、風府、神門、太沖穴治療腦卒中后抑郁有效，可以增加體重、糖水消耗量以及提高垂直運動次數(shù)和水平運動次數(shù)，且較鹽酸氟西汀治療腦卒中后抑郁，療效無明顯差異。通過本實驗研究證實針刺百會、風府、神門、太沖穴是防治腦卒中后抑郁大鼠的有效治療手段，具有研究和進一步開發(fā)的價值。
[Abstract]:Objective: to observe and analyze the effects of acupuncture at Baihui, Fengfu, Shenmen and Taicong points on the behavior, nerve function defect and body weight of rats with post-stroke depression and provide scientific basis for acupuncture and moxibustion treatment of post-stroke depression. Methods: the middle cerebral artery occlusion model was used. Rats were treated with chronic mild unpredictable stress (CUMS) on the 7th day after waking to establish a complex model of stroke with depression. The Longa scoring method was used to evaluate the neurological deficit and recovery of stroke rats, and the changes of interest and pleasure of PSD rats were evaluated by sugar water consumption test. Open box experiment was used to evaluate the degree of curiosity and the changes of spontaneous motion in PSD rats. Results: after 2 hours of MACO model was established, the neurological impairment scores were evaluated in rats. The Longa scores of the rats in each group were 0 after 1-3 minutes and 24 hours after the neural function defect was basically restored to normal. The results showed that the rats had mild dysfunction of contralateral limbs and obvious neurological impairment after operation. The results of body weight and behavioral evaluation were compared between the blank group and the model group. The results of body weight and behavioral evaluation in the model group were significantly lower than that in the control group (P < 0.05), indicating that the PSD model was successful. After treatment, the results of body weight and behavior evaluation in the acupuncture group and the western medicine group showed that the curative effect of the two groups was significant (P < 0.05), but the difference between the two groups was not significant (P > 0.05). Conclusion: the model of middle cerebral artery occlusion combined with chronic unpredictable stress depression can be successfully used to establish the animal model of post-stroke depression. Acupuncture at Baihui, Fengfu, Shenmen and Taicong points is effective in treating post-stroke depression. It can increase body weight, consumption of sugar and water, increase the number of vertical and horizontal movements, and is more effective than fluoxetine hydrochloride in treating post-stroke depression. There was no significant difference in curative effect. This experiment proved that acupuncture at Baihui, Fengfu, Shenmen and Taichong is an effective therapeutic method for prevention and treatment of post-stroke depression in rats, which has the value of research and further development.
