【摘要】：目的:觀察硬膜外阻滯麻醉鎮(zhèn)痛對(duì)產(chǎn)婦分娩的影響。方法:將197例產(chǎn)婦按隨機(jī)數(shù)字表法分為觀察組(95例)和對(duì)照組(102例)。對(duì)照組產(chǎn)婦自然分娩,觀察組在對(duì)照組基礎(chǔ)上加用硬膜外阻滯麻醉鎮(zhèn)痛。比較兩組產(chǎn)婦的新生兒阿氏(Apgar)評(píng)分(5min)、行為神經(jīng)評(píng)分及產(chǎn)婦視覺模擬(VAS)評(píng)分,母嬰結(jié)局及不良反應(yīng)發(fā)生率。結(jié)果:兩組產(chǎn)婦的新生兒Apgar評(píng)分及行為神經(jīng)評(píng)分比較,差異無統(tǒng)計(jì)學(xué)意義(P0.05);觀察組產(chǎn)婦VAS評(píng)分及剖宮產(chǎn)率顯著低于對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05);觀察組產(chǎn)婦產(chǎn)后出血、陰道助產(chǎn)及新生兒窒息率比較,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組產(chǎn)婦皮膚瘙癢、惡心嘔吐、頭痛、下肢發(fā)麻等不良反應(yīng)經(jīng)對(duì)癥處理后好轉(zhuǎn)。結(jié)論:硬膜外阻滯麻醉用于分娩鎮(zhèn)痛對(duì)新生兒無影響,能明顯減輕產(chǎn)婦疼痛,且安全性較好。
[Abstract]:Objective: to observe the effect of epidural block anesthesia and analgesia on parturient delivery. Methods: 197 cases of parturient were randomly divided into observation group (n = 95) and control group (n = 102). Control group parturient natural delivery, observation group on the basis of the control group with epidural block anesthesia analgesia. The neonatal Apgara score, behavioral nerve score, visual analogue vas score, maternal and infant outcome and incidence of adverse reactions were compared between the two groups. Results: there was no significant difference in neonatal Apgar score and behavioral nerve score between the two groups, while the VAS score and cesarean section rate in the observation group were significantly lower than those in the control group (P 0.05). There was no significant difference in the rate of vaginal midwifery and neonatal asphyxia (P 0.05). The side effects of skin itching, nausea and vomiting, headache and lower limb anaesthesia were improved in the observation group. Conclusion: epidural block anesthesia has no effect on neonatal analgesia, and it can obviously relieve the pain of parturient.
