種植體與天然牙聯(lián)合支持固定橋的生物力學分析

【摘要】:背景:天然牙和種植體應力分布模式不盡相同,種植體-天然牙聯(lián)合支持式固定橋的應力分布模式有待探討。目的:對種植體、天然牙單冠以及種植體-天然牙聯(lián)合支持固定橋進行垂直載荷,分析種植體骨界面與天然牙牙周的應力、應變分布情況。方法:選用2只成年雄性Beagle犬,全麻下拔除2只犬右側下頜第一磨牙,3個月后拔牙區(qū)植入1顆Straumann 3.3 mm×10 mm常規(guī)頸種植體。3個月后麻醉處死,去處軟組織后取完整下頜骨,一只犬行右側下頜第四雙根牙單冠和種植體單冠修復,另一只犬行聯(lián)冠修復。下頜骨采用自凝塑料包埋后固定在加載裝置上,在種植體和天然牙頸部相對應的頰舌側皮質骨上貼應變片,并貼相應的電阻補償片。在種植體、天然牙單冠以及種植體-天然牙固定橋基牙咬合面上分別采用40,60,80,120及160 N的力進行垂直加載。結果與結論:(1)種植體天-然牙聯(lián)合修復后,當加載在種植體上時,其應變值要小于加載在種植體單冠上的應變值,部分負荷為天然牙承擔;當加載在天然牙上時,其應變值要小于加載在天然牙單冠的應變值,部分負荷為種植體承擔;(2)種植體-天然牙聯(lián)合固定橋時,加載在天然牙基牙上時,應力由種植體和天然牙共同承擔;而加載在種植體上基牙時,應力基本由種植體承擔;(3)提示臨床上種植體-天然牙聯(lián)合固定橋修復是可行的。
[Abstract]:Background: the stress distribution patterns of natural teeth and implants are different. Objective: to analyze the distribution of stress and strain between implant bone interface and periodontal region of natural teeth by vertical loading on implants, natural tooth monocoronals and implant-natural tooth support fixation bridges. Methods: two adult male Beagle dogs were selected and the right first mandibular molars were extracted under general anesthesia. A Straumann 3.3 mm 脳 10 mm conventional cervical implant was implanted in the extraction area 3 months later. After 3 months of anesthesia, the intact mandible was removed from the soft tissue. One dog was repaired with the fourth double crown of right mandible and single crown of implant, the other dog was repaired with combined crown. The mandible was immobilized on the loading device with self-coagulating plastic and the strain film was attached to the cortical bone of the buccal and lingual side corresponding to the implant and the natural tooth neck and the corresponding resistive compensation film was applied to the mandible. The occlusal surfaces of implants, natural dental crowns and implant-natural fixed abutments were subjected to vertical loading with force of 40,60,80120 and 160N, respectively. Results and conclusion (1) when the implant was loaded on the implant, the strain value was smaller than the strain value on the single crown of the implant, part of the load was borne by the natural tooth, and when the implant was loaded on the natural tooth, the strain value of the implant was less than that of the implant. The strain value should be less than the strain value of the natural tooth under single crown, and the stress of the implant and the natural tooth should be borne by both the implant and the natural tooth when it is loaded on the natural abutment when the partial load is the implant-bearing joint fixation bridge. When the abutment is loaded on the implant, the stress is basically borne by the implant. 3) it is suggested that the implant-natural tooth fixation bridge is feasible in clinic.
