【摘要】：目的探討氣囊仿生助產(chǎn)與自控硬膜外麻醉應用于產(chǎn)科的臨床效果。方法選取2013年4月至2015年4月128例初產(chǎn)婦,根據(jù)產(chǎn)婦自愿選擇的原則,將其分為觀察組和對照組,其中觀察組64例給予氣囊仿生助產(chǎn)術聯(lián)合自控硬膜外麻醉進行分娩;對照組64例,自然臨產(chǎn)不采用任何醫(yī)療干預措施。比較兩組產(chǎn)婦的鎮(zhèn)痛效果、產(chǎn)程、分娩方式以及母嬰結(jié)果。結(jié)果觀察組產(chǎn)婦在分娩開始30 min、1 h后的鎮(zhèn)痛效果明顯優(yōu)于對照組,差異有統(tǒng)計學意義(P0.05)。觀察組產(chǎn)婦產(chǎn)程的活躍期、第二產(chǎn)程順產(chǎn)率、剖宮產(chǎn)率和新生兒窒息率等均優(yōu)于對照組,差異有統(tǒng)計學意義(P0.05),兩組第三產(chǎn)程、產(chǎn)后出血量比較差異未見統(tǒng)計學意義(P0.05)。結(jié)論氣囊仿生助產(chǎn)聯(lián)合自控硬膜外麻醉,可有效減輕分娩疼痛,縮短產(chǎn)程,提高陰道分娩率,降低剖宮產(chǎn)率,且對產(chǎn)婦和新生兒的影響小,安全可靠,具有重要的臨床應用價值。
[Abstract]:Objective to investigate the clinical effect of balloon bionic midwifery and self-controlled epidural anesthesia in obstetrics. Methods 128 primiparae from April 2013 to April 2015 were divided into observation group and control group according to the principle of voluntary choice of puerpera. 64 cases in observation group were given pneumatic bionic midwifery combined with self-controlled epidural anesthesia for delivery. In the control group, 64 cases of spontaneous labor were not treated with any medical intervention. The analgesic effect, labor process, delivery mode and maternal and infant outcome were compared between the two groups. Results the analgesic effect of the observation group was significantly better than that of the control group 30 min after delivery and the difference was statistically significant (P 0.05). The active stage, the second stage of labor, the rate of cesarean section and the rate of neonatal asphyxia in the observation group were better than those in the control group, and the difference was statistically significant (P 0.05). There was no significant difference between the two groups in the third stage of labor and the amount of postpartum hemorrhage. Conclusion Balloon bionic midwifery combined with self-controlled epidural anesthesia can effectively reduce labor pain, shorten labor process, increase vaginal delivery rate, reduce cesarean section rate, and have little effect on parturient and newborn, and it is safe and reliable. It has important clinical application value.
