外周血T細(xì)胞及其亞群與末端回腸炎關(guān)系的研究

【摘要】:目的:探討外周血T淋巴細(xì)胞及其亞群構(gòu)成比例在末端回腸炎發(fā)病過(guò)程中的變化及其意義。方法:取SD大鼠90只。體重均為200至350g。將大鼠隨機(jī)分為M、S、C三組。M組:模型組(Model);S組:縫線組(Suture);C組:對(duì)照組(Control)。每組30只。三組分別采取不同的處理。M組:回腸遠(yuǎn)端與結(jié)腸近端吻合,制造結(jié)腸-回腸反流模型。S組:制造與M組在回腸和結(jié)腸相同位置的同等程度的腸道損傷,而不建立反流模型,形成對(duì)照。C組:僅麻醉,不行手術(shù)。三組大鼠飼養(yǎng)及護(hù)理均相同。分別于術(shù)后第2、4、8周,每組隨機(jī)抽取10只大鼠進(jìn)行靜脈采血及取回腸組織。靜脈血用流式細(xì)胞術(shù)檢測(cè)T細(xì)胞亞群。回腸組織制成切片進(jìn)行病理評(píng)分。結(jié)果:1.外周血T細(xì)胞亞群構(gòu)成比例檢測(cè):(1)實(shí)驗(yàn)大鼠DNT細(xì)胞構(gòu)成比例情況:術(shù)后M組、S組大鼠DNT細(xì)胞構(gòu)成比例均明顯增加,M組更明顯(P0.05),到第2周時(shí)達(dá)到高峰,說(shuō)明DNT可能參與了急性炎癥反應(yīng)過(guò)程。從第2周開(kāi)始至第八8周數(shù)值逐漸下降,S組在第4周時(shí)基本恢復(fù)正常值,M組較S組慢,M組與S組比較有顯著差異(P0.05)。說(shuō)明DNT細(xì)胞在實(shí)驗(yàn)?zāi)┒嘶啬c炎急性期(0-4周)變化最明顯。(2)實(shí)驗(yàn)大鼠T4細(xì)胞構(gòu)成比例情況:術(shù)后M組、S組大鼠T4細(xì)胞構(gòu)成比均明顯降低,m組表現(xiàn)更明顯(p0.05),術(shù)后第2周數(shù)值最低值。術(shù)后第2周開(kāi)始,s組大鼠t4細(xì)胞構(gòu)成比開(kāi)始增加,到第4周時(shí)接近正常水平,而同一時(shí)期m組維持在低值水平,有少數(shù)數(shù)值甚至比第2周時(shí)更低,4周以后開(kāi)始回升到正常值水平。說(shuō)明末端回腸炎的早期(0-4周),t4細(xì)胞可能扮演了重要的角色。(3)實(shí)驗(yàn)大鼠t8細(xì)胞構(gòu)成比例情況:術(shù)后2周內(nèi),m組、s組大鼠t8細(xì)胞構(gòu)成比例均增加,m組、s組有顯著差異(p0.05)。第4周時(shí),m組t8細(xì)胞構(gòu)成比例進(jìn)一步升高,而s組基本達(dá)到正常水平(p0.05)。第8周時(shí),m組恢復(fù)正常水平。說(shuō)明末端回腸炎發(fā)病的4-8周內(nèi),t8細(xì)胞可能起了著重要的作用。(4)實(shí)驗(yàn)大鼠dpt細(xì)胞構(gòu)成比例情況:術(shù)后m組大鼠dpt細(xì)胞構(gòu)成比例明顯增加,第4周達(dá)到高峰,4-8周逐漸下降,到第8周時(shí)仍高于c組,而s組dpt細(xì)胞數(shù)量變化始終不明顯,m、s組有顯著差異(p0.05)。結(jié)果提示,dpt細(xì)胞可能參與了末端回腸炎的發(fā)病整個(gè)過(guò)程(0-8周)。2.外周血t細(xì)胞亞群構(gòu)成比例與回腸組織炎癥程度的相關(guān)性:通過(guò)對(duì)m組術(shù)后病變明顯處組織的肉眼和鏡下觀察發(fā)現(xiàn),在實(shí)驗(yàn)性末端回腸的整個(gè)發(fā)病過(guò)程中,隨著時(shí)間延長(zhǎng),病變處腸組織病理評(píng)分越高,炎癥表現(xiàn)越嚴(yán)重。通過(guò)m組和c組各t細(xì)胞亞群構(gòu)成比與炎癥評(píng)分的相關(guān)性分析表明,dnt、t8、dpt三類細(xì)胞與末端回腸組織炎癥嚴(yán)重程度具有正相關(guān)性(r值分別為0.518、0.482、0.714,p0.05),t4細(xì)胞與末端回腸組織炎癥嚴(yán)重程度具有負(fù)相關(guān)性(r=-0.506,P0.05)。結(jié)論:1.在ETI中,DNT、T8、DPT三類T細(xì)胞亞群增加,T4細(xì)胞減少,外周血T細(xì)胞各亞群可能均參與了ETI的發(fā)病過(guò)程。2.DNT、T8、DPT三類細(xì)胞與末端回腸組織炎癥嚴(yán)重程度具有正相關(guān)性,T4細(xì)胞與末端回腸組織炎癥嚴(yán)重程度具有負(fù)相關(guān)性,T細(xì)胞各亞群可能作為監(jiān)測(cè)末端回腸炎癥程度的重要指標(biāo)。
[Abstract]:Objective: To investigate the changes and significance of the proportion of T lymphocyte and its subgroup in peripheral blood and its subgroup in the pathogenesis of terminal ileocolitis. Methods: 90 SD rats were randomly divided into M, S, C three groups.M groups: model group (Model); S group: suture group (Suture); C group (Control), 30. Three groups were different respectively. The treatment group.M: the distal ileum and the proximal colon anastomosis, the colonic ileum reflux model.S group: the same degree of intestinal injury in the same position of the ileum and colon in the M group, but not the reflux model, the control group of.C: only anesthesia, no operation. The three groups of rats feeding and nursing were the same. Each group followed 2,4,8 weeks after the operation, respectively. Blood sampling and intestinal tissue were taken in 10 rats. The T cell subgroup was detected by flow cytometry. The ileum tissue was made into slices for pathological score. Results: the proportion of T cell subgroups in peripheral blood was detected in 1. peripheral blood: (1) the proportion of DNT cells in experimental rats: M group after operation, the proportion of DNT cells in group S increased obviously, M The group was more obvious (P0.05) and reached the peak at the second week, indicating that DNT might be involved in the acute inflammatory reaction process. From the beginning of second weeks to eighth 8 weeks, the value of the group S was gradually reduced, and the group M was basically recovered at fourth weeks. The group M was slower than the S group, and there was a significant difference between the group of M and the S group (P0.05). The changes showed that the DNT cell at the acute stage of the experimental terminal colitis (0-4 weeks) was changed. Most obvious. (2) the proportion of T4 cells in experimental rats: the ratio of T4 cells in group M, group S and group M was significantly lower than that in group M (P0.05), and the lowest value was at the beginning of the operation. The constitution ratio of T4 cells in group s increased and reached the normal level at fourth weeks, while the M group maintained at a low level at the same time, and there was a low level in the same period. There was a low level in the same period. A few values were even lower than second weeks and began to rise to normal levels after 4 weeks. It was suggested that T4 cells may play an important role in the early stage of terminal colitis (0-4 weeks). (3) the proportion of T8 cells in experimental rats: the proportion of T8 cells in group M and Group s increased in 2 weeks after operation, and in group M, s group had significant difference (P0.05). Fourth weeks. The proportion of T8 cells in the M group increased further, while the s group basically reached the normal level (P0.05). At eighth weeks, the M group returned to the normal level. It indicated that the T8 cells may play an important role in the pathogenesis of terminal ileocolitis. (4) the proportion of DPT cells in experimental rats: the proportion of DPT cells in the group M rats increased significantly after fourth weeks, reaching fourth weeks. At the peak, the 4-8 week decreased gradually and was still higher than the C group at the eighth week, but the number of DPT cells in group s was not significantly changed, and there was a significant difference in M and s group (P0.05). The results suggest that DPT cells may participate in the whole process of terminal ileocolitis (0-8 weeks) and the correlation between the proportion of T lymphocyte subsets in the peripheral blood of.2. and the degree of inflammation in the ileum tissue: through M In group M and C group, the pathological score of the intestinal tissue was higher and the inflammation was more serious in the whole process of the experimental terminal ileum. The correlation between the constituent ratio of T cell subsets and the inflammation score in the group of M and C groups showed that the DNT, T8, DPT were the cells and the end. There is a positive correlation between the inflammatory severity in the end of the ileum (r value is 0.518,0.482,0.714, P0.05), and there is a negative correlation between the inflammatory severity of the T4 cells and the terminal ileum (r=-0.506, P0.05). Conclusion: 1. in ETI, DNT, T8, DPT three T cell subsets increase, T4 cells decrease, and the subgroups of peripheral blood cells may be involved in the pathogenesis of the disease. Process.2.DNT, T8, DPT three cells are positively correlated with the severity of the inflammation in the terminal ileum. The T4 cells are negatively correlated with the severity of the terminal ileum, and the T cell subgroups may be an important indicator for monitoring the degree of terminal ileocolitis.
