自體紅骨髓一期移植治療股骨粉碎型骨折

【摘要】:背景:骨折愈合是否需要一期紅骨髓移植,是否植入間充質(zhì)干細(xì)胞?當(dāng)股骨粉碎性骨折時(shí)進(jìn)行閉式復(fù)位髓內(nèi)釘內(nèi)固定術(shù),同期進(jìn)行自體紅骨髓一期移植是否可促進(jìn)骨折處成骨的作用,尚處于研究階段。目的:探討采取閉式復(fù)位髓內(nèi)釘內(nèi)固定方法,一期治療同時(shí)給予自體紅骨髓移植對(duì)骨折愈合的影響。方法:選擇健康新西蘭兔20只,麻醉后以30 cm高度質(zhì)量為5 kg重錘自由下落的做功條件制作兔股骨粉碎型骨折模型。給予2.0 mm直徑克氏針?biāo)鑳?nèi)固定系統(tǒng)模仿股骨髓內(nèi)釘內(nèi)固定術(shù)對(duì)其進(jìn)行固定。將兔股骨粉碎性骨折內(nèi)固定造模成功的右側(cè)股骨定義為骨折內(nèi)固定組,將其左股骨定義為紅骨髓移植組。紅骨髓移植組將提取的1 mL自體紅骨髓注射于骨折處。于造模后第14,28天分批處死10只兔,測(cè)量X射線片上骨折周圍骨痂最厚處骨痂厚度;蘇木精-伊紅染色后光鏡觀察2組骨折處組織切片中軟骨細(xì)胞數(shù)量及骨小梁相對(duì)面積。結(jié)果與結(jié)論:(1)骨折后14 d,骨折內(nèi)固定組較紅骨髓移植組骨折周圍骨痂生成厚度明顯減小(P0.01),差異有顯著性意義;(2)骨折后14 d,紅骨髓移植組骨折處軟骨細(xì)胞生成數(shù)量大于骨折內(nèi)固定組(P0.01),骨折愈合速度快于骨折內(nèi)固定組;(3)骨折后28 d,骨折內(nèi)固定組較紅骨髓移植組骨折處骨小梁相對(duì)面積明顯縮小(P0.01);(4)結(jié)果提示,紅骨髓移植對(duì)于閉式復(fù)位髓內(nèi)釘內(nèi)固定修復(fù)兔股骨干粉碎型骨折的愈合具有促進(jìn)作用。
[Abstract]:Background: does one stage red bone marrow transplantation and mesenchymal stem cells are needed for fracture healing? It is still in the research stage that closed reduction and intramedullary nail internal fixation and autologous red bone marrow transplantation at the same time can promote the osteogenesis of the fracture when the femoral comminuted fracture is treated with closed reduction and intramedullary nail fixation. Objective: to investigate the effect of closed reduction and intramedullary nail fixation and autologous red bone marrow transplantation on fracture healing. Methods: 20 healthy New Zealand rabbits were used to establish the femoral comminuted fracture model after anesthesia. A 2.0 mm diameter Kirschner needle fixation system was given to imitated femoral bone marrow nail fixation. The right femur was defined as the fracture internal fixation group and the left femur was defined as the red bone marrow transplantation group. In the red bone marrow transplantation group, 1 mL autologous red bone marrow was injected into the fracture. Ten rabbits were killed in batches on the 28th day after modeling, and the thickness of callus around fracture was measured, and the number of chondrocytes and the relative area of trabecular bone were observed by light microscope after hematoxylin-eosin staining. Results and conclusion at 14 days after fracture, the thickness of callus formation around the fracture in the internal fixation group was significantly reduced than that in the red bone marrow transplantation group, and the difference was significant (P 0.01). The number of chondrocytes in the fracture site of the red bone marrow transplantation group was significantly lower than that in the red bone marrow transplantation group at 14 days after the fracture. The fracture healing speed was faster than that in the fracture internal fixation group 28 days after the fracture. The relative area of trabecular bone in the fracture internal fixation group was significantly smaller than that in the red bone marrow transplantation group, and the results suggested that the bone trabecular area of the fracture group was significantly smaller than that of the red bone marrow transplantation group. Red bone marrow transplantation can promote the healing of femoral shaft comminuted fracture in rabbits with closed reduction and intramedullary nail fixation.
