【摘要】：目的探討ORM1 A113G基因多態(tài)性對(duì)女性患者維庫(kù)溴銨肌松效應(yīng)的影響。方法選擇擇期全麻下乳腺包塊切除術(shù)貴州漢族女性患者107例,年齡18~65歲。術(shù)前采集外周靜脈血5ml,使用天根血液基因組DNA提取試劑盒抽取DNA。采用3730XL DNA測(cè)序儀直接測(cè)序?qū)RM1A113G進(jìn)行基因分型。麻醉誘導(dǎo)期間給予維庫(kù)溴銨0.1mg/kg,用TOF-Watch加速度儀進(jìn)行肌松監(jiān)測(cè)。觀察起效時(shí)間、時(shí)效、體內(nèi)作用時(shí)間、恢復(fù)指數(shù)、TOF恢復(fù)到25%的時(shí)間。結(jié)果根據(jù)基因型將患者分為野生型純合子組(AA組,n=71)、突變型雜合子組(AG組,n=35)和突變型純合子組(GG組,n=1)。將AG組和GG組兩種基因型合并為變異組。與AA組比較,變異組的時(shí)效和TOF恢復(fù)到25%時(shí)間均明顯延長(zhǎng)(P0.05);起效時(shí)間、恢復(fù)指數(shù)和體內(nèi)作用時(shí)間差異無統(tǒng)計(jì)學(xué)意義。結(jié)論ORM1 A113G基因多態(tài)性可在一定程度上影響貴州漢族女性患者維庫(kù)溴銨的肌松效應(yīng)。
[Abstract]:Objective to investigate the effect of polymorphism of ORM1 A 113G gene on muscle relaxation of vecuronium bromide in female patients. Methods 107 female patients, aged 1865 years, underwent breast mass resection under elective general anesthesia in Guizhou Han nationality. The peripheral venous blood was collected 5 ml before operation and the genomic DNA extraction kit was used to extract DNA. The genotyping of ORM1A113G was carried out by direct sequencing with 3730XL DNA sequencer. Vecuronium bromide was given 0.1 mg / kg during anesthesia induction, and muscle relaxation was monitored by TOF-Watch accelerometer. The response time, time of action, time of action in vivo and recovery index of TOF were observed to be 25%. Results the patients were divided into wild type homozygote group (AA group), mutant heterozygote group (AG group) and mutant homozygote group (GG group). AG and GG genotypes were combined into variant group. Compared with AA group, the time of time and TOF recovery to 25% in the mutant group were significantly prolonged (P 0.05), but there was no significant difference in the onset time, recovery index and in vivo action time. Conclusion the polymorphism of ORM1 A113G gene may affect the muscle relaxation effect of vecuronium in Guizhou Han women.
