【摘要】：目的探討地佐辛與氟比洛芬酯對乳腺癌圍術期鎮(zhèn)痛效果與免疫抑制的影響。方法選取該院外科乳腺癌擇期手術患者76例,在相同麻醉誘導和維持用藥基礎上,采用隨機數(shù)字表法將患者分為觀察組(n=40)和對照組(n=36),觀察組患者麻醉誘導前給予地佐辛,對照組患者麻醉誘導前給予氟比洛芬酯,比較兩組患者圍術期鎮(zhèn)痛效果和免疫抑制的影響。結果觀察組術后3.0、6.0、12.0h視覺模擬疼痛評分(VAS)和Ramsay鎮(zhèn)靜評分明顯低于對照組,兩組比較差異有統(tǒng)計學意義(P0.05)。麻醉前,T淋巴細胞亞群(CD4+、CD8+、CD4+/CD8+)和NK細胞(CD16+CD56+)的組間比較差異無統(tǒng)計學意義(P0.05),觀察組手術開始1.5h時,CD16+CD56+明顯高于對照組,手術結束后24.0h,CD4+、CD4+/CD8+和CD16+CD56+明顯高于對照組,兩組比較差異有統(tǒng)計學意義(P0.05),且手術開始1.5h、手術結束后24.0h的上述指標均明顯低于麻醉前,不同時段比較差異有統(tǒng)計學意義(P0.05),兩組患者術后不良反應發(fā)生率比較差異無統(tǒng)計學意義(P0.05)。結論地佐辛對乳腺癌圍術期鎮(zhèn)痛效果和免疫抑制預防作用明顯優(yōu)于氟比洛芬酯。
[Abstract]:Objective to investigate the effect of dizosin and flurbiprofen on perioperative analgesia and immunosuppression of breast cancer.Methods Seventy-six patients with breast cancer undergoing elective operation were divided into two groups: observation group (n = 40) and control group (n = 36), on the basis of the same anesthetic induction and maintenance medication. The patients in the observation group were given dizosin before anesthesia induction.Patients in the control group were treated with flurbiprofen before anesthesia induction, and the analgesic effect and immunosuppressive effect were compared between the two groups.Results the visual analogue pain score (VASS) and Ramsay sedation score in the observation group were significantly lower than those in the control group (P 0.05).There was no significant difference between the T lymphocyte subsets (CD4 / CD8 / CD8) and NK cell CD16 CD56 (P 0.05). At 1.5 h after operation, the CD16 CD56 in the observation group was significantly higher than that in the control group, and the ratio of CD4 / CD8 and CD16 CD56 in the observation group was significantly higher than that in the control group at 24.0h after operation.The difference between the two groups was statistically significant (P 0.05), and the above indexes were significantly lower than those before anesthesia at the beginning of operation 1.5 hours and 24.0h after operation.There was no significant difference in the incidence of postoperative adverse reactions between the two groups (P 0.05).Conclusion the analgesic effect and immunosuppressive effect of dizosin on breast cancer is better than flurbiprofen ester.
