【摘要】：目的探討氣管插管全麻術(shù)患者術(shù)后發(fā)生下呼吸道感染的危險因素及相關(guān)預(yù)防治療措施。方法回顧性分析2010年9月-2014年11月在醫(yī)院行氣管插管全麻術(shù)的675例患者臨床資料,其中50例患者術(shù)后發(fā)生下呼吸道感染,分析患者發(fā)生下呼吸道感染的相關(guān)因素;并進一步通過多因素logistic回歸分析患者發(fā)生下呼吸道感染的危險因素。結(jié)果 675例行氣管插管全麻術(shù)患者,術(shù)后發(fā)生下呼吸道感染50例,感染率7.4%;下呼吸道感染患者的痰液培養(yǎng)出31株病原菌,其中革蘭陰性菌25株占80.6%,革蘭陽性菌5株占16.2%,真菌1株占3.2%;單因素分析顯示,年齡≥50歲、有吸煙史、麻醉時間3h、合并基礎(chǔ)疾病、插管部位、插管途徑、拔管時間延遲以及插管深度與下呼吸道感染的發(fā)生明顯相關(guān),且差異有統(tǒng)計學(xué)意義(P0.05);進一步多因素分析顯示,年齡≥50歲、麻醉時間3h、插管途徑、插管部位、插管深度以及拔管時間延遲是患者發(fā)生下呼吸道感染的獨立危險因素(P0.05)。結(jié)論臨床上應(yīng)該加強對該危險因素的控制,以降低患者發(fā)生下呼吸道感染的風(fēng)險。
[Abstract]:Objective to investigate the risk factors of lower respiratory tract infection in patients undergoing tracheal intubation and general anesthesia.Methods the clinical data of 675 patients undergoing general anesthesia with tracheal intubation in hospital from September 2010 to November 2014 were analyzed retrospectively. Among them, 50 cases developed lower respiratory tract infection after operation, and the related factors of lower respiratory tract infection were analyzed.The risk factors of lower respiratory tract infection were further analyzed by multivariate logistic regression analysis.Results among 675 patients undergoing tracheal intubation and general anesthesia, 50 cases of lower respiratory tract infection occurred after operation, the infection rate was 7.4%, 31 strains of pathogenic bacteria were cultured in sputum of patients with lower respiratory tract infection.Among them, 25 Gram-negative bacteria accounted for 80.6, 5 Gram-positive bacteria accounted for 16.2and 1 fungus accounted for 3.2.The univariate analysis showed that the age 鈮，
