【摘要】：目的:比較麻醉方法和術(shù)后鎮(zhèn)痛方式對(duì)胃癌患者手術(shù)后下呼吸道感染發(fā)生率的影響。方法:選取2013年1月至2015年2月收治的94例胃癌患者,根據(jù)麻醉方法與鎮(zhèn)痛方式的差異,分為研究組和對(duì)照組。每組各47例患者,研究組采用硬膜外復(fù)合全麻并于手術(shù)后應(yīng)用硬膜外自控鎮(zhèn)痛(PCEA)。對(duì)照組則采用傳統(tǒng)的全身麻醉與PCVA(靜脈自控鎮(zhèn)痛)。于手術(shù)后對(duì)兩組患者下呼吸道感染的發(fā)生率進(jìn)行觀察分析。結(jié)果:研究組和對(duì)照組手術(shù)后下呼吸道感染發(fā)生率分別為10.64%(5例)和23.40%(11例)。研究組顯著低于對(duì)照組,且研究組p H值、Pa O2以及SO2指標(biāo)明顯優(yōu)于對(duì)照組,組間數(shù)據(jù)差異明顯,具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:硬膜外復(fù)合全麻麻醉并用PCEA的麻醉、鎮(zhèn)痛方法,可有效降低下呼吸道感染的發(fā)生率,顯著提高患者的治療效果與生活質(zhì)量,值得推廣與應(yīng)用。
[Abstract]:Objective: to compare the effect of anaesthesia and postoperative analgesia on the incidence of lower respiratory tract infection in patients with gastric cancer.Methods: 94 patients with gastric cancer from January 2013 to February 2015 were divided into study group and control group according to the difference of anesthesia and analgesia.There were 47 patients in each group. The study group was treated with epidural combined general anesthesia and PCEA after operation.The control group was treated with traditional general anesthesia and PCVA (patient-controlled intravenous analgesia).The incidence of lower respiratory tract infection in the two groups was observed and analyzed after operation.Results: the incidence of lower respiratory tract infection in the study group and the control group was 10.64 and 23.40 respectively.The values of pH, Pao 2 and SO2 in the study group were significantly lower than those in the control group, and the difference between the two groups was significant (P 0.05).Conclusion: combined epidural combined general anesthesia with PCEA anesthesia and analgesia can effectively reduce the incidence of lower respiratory tract infection and improve the treatment effect and quality of life of the patients. It is worth popularizing and applying.
