【摘要】：目的:通過(guò)比較壓力控制通氣模式(pressure control ventilation,PCV)與壓力控制容量保證通氣模式(pressure controlled ventilation-volume guaranteed,PCV-VG)對(duì)胸外科手術(shù)需行單肺通氣的患者呼吸力學(xué)、動(dòng)脈氧合及炎性因子釋放的影響,來(lái)探討PCV-VG模式的在單肺通氣中的應(yīng)用。方法:選擇60例擇期行肺葉切除術(shù)或者食管癌根治術(shù)的患者,年齡45至76歲,ASA分級(jí)Ⅰ~Ⅱ級(jí),排除有嚴(yán)重心腦血管疾病、肝腎功能不全者,要求近期無(wú)上呼吸道感染,戒煙大于4周,術(shù)前肺功能檢查第一秒用力呼氣量(FEV1)與用力肺活量(FVC)之比50%。采用隨機(jī)數(shù)字法將所有患者分為兩組,即PCV組(P組,n=30)與PCV-VG組(G組,n=30)。麻醉誘導(dǎo)后手術(shù)進(jìn)胸前均采用容量控制通氣模式(volume control ventilation,VCV),單肺通氣時(shí),P組采用PCV模式,G組采用PCV-VG模式。分別于插管后(T1)、單肺通氣即刻(T2)、單肺通氣30min(T3)、單肺通氣60min(T4)、恢復(fù)雙肺通氣30min時(shí)(T5)記錄患者的肺順應(yīng)性(CL)、氣道峰壓(Ppeak)、氣道平均壓(Pmean)和氣道阻力(RAW),并抽動(dòng)脈血行血?dú)夥治?記錄動(dòng)脈血氧分壓(PO2)、二氧化碳分壓(PCO2)及乳酸(Lac)值,在T2~T5及術(shù)后24小時(shí)(T6)測(cè)定血漿中白介素-6(IL-6)、白介素-10(IL-10)、腫瘤壞死因子(TNF-α)濃度。結(jié)果:與T1時(shí)比較:兩組患者CL在T2~T4時(shí)明顯降低、PO2在T3~T4時(shí)顯著下降(P(27)0.05或P(27)0.01),Pmean與RAW在T2~T5時(shí)明顯升高、PCO2在T5時(shí)明顯升高(P(27)0.05或P(27)0.01);P組患者Ppeak在T2~T5時(shí)升高、動(dòng)脈血Lac值在T5時(shí)升高(P(27)0.05或P(27)0.01),而G組無(wú)明顯變化。與T2時(shí)比較:T3~T6時(shí)兩組患者血漿IL-6、TNF-α水平升高(P(27)0.01);IL-10在T3~T5時(shí)升高(P(27)0.01)。與P組相比,G組患者在T3~T4時(shí)IL-10水平升高(P(27)0.01),T6時(shí)IL-6水平降低(P(27)0.05)。結(jié)論:單肺通氣期間采用PCV-VG模式呼吸力學(xué)變化更為穩(wěn)定,且在一定程度上可降低炎性反應(yīng),有利于患者肺功能保護(hù)。
