【摘要】：目的探索針刺輔助麻醉用于行甲狀腺部分切除術(shù)中對于患者血流動力學(xué)的影響。方法在我院2014年6月至2016年6月間行甲狀腺部分切除術(shù)患者中隨機(jī)抽取出80例作為觀察對象,根據(jù)麻醉方法的不同將其分入到觀察組和對照組中,其中對照組患者給予頸叢局部浸潤麻醉復(fù)合杜氟合劑,觀察組患者給予針刺麻醉+靶控輸注丙泊酚和瑞芬太尼麻醉,對比分析兩組患者在不同時間點(diǎn)的平均動脈壓、心率指標(biāo)、睜眼時間、清醒時間、PACU停留時間、不良反應(yīng)發(fā)生率等指標(biāo)。結(jié)果觀察組患者T3、T4時間點(diǎn)的HR和MAP值均低于對照組,P0.05;觀察組患者的睜眼時間、清醒時間、PACU停留時間比對照組短,不良反應(yīng)發(fā)生率2.38%低于對照組的18.42%,P0.05。結(jié)論在甲狀腺部分切除術(shù)的麻醉中采用針刺輔助靶控輸注丙泊酚和瑞芬太尼麻醉效果好,對患者的血流動力學(xué)影響小,患者術(shù)后清醒快,安全性高,值得推廣應(yīng)用。
[Abstract]:Objective to explore the effect of acupuncture assisted anesthesia on hemodynamics in patients undergoing partial thyroidectomy. Methods 80 patients with partial thyroidectomy were randomly selected from our hospital from June 2014 to June 2016. According to the different methods of anesthesia, they were divided into the observation group and the control group. The patients in the control group were given local infiltration anesthesia of cervical plexus combined with Duflon mixture, and the patients in the observation group were anesthetized with target-controlled infusion of propofol and remifentanil under acupuncture anesthesia. The mean arterial pressure, heart rate, eye opening time, waking time and PACU residence time were compared between the two groups at different time points. Results the HR and MAP values of the patients in the observation group were lower than those in the control group (P 0.05), and the time of opening eyes, waking time and staying time of PACU in the observation group were shorter than those in the control group. The incidence of adverse reactions was 2.38% lower than that of the control group (18.42%, P 0.05.Conclusion Acupuncture assisted target-controlled infusion of propofol and remifentanil is effective in the anesthesia of partial thyroidectomy, which has little effect on hemodynamics, and the patients wake up quickly after operation. High safety, worth popularizing and applying.
