丙泊酚復合硬膜外麻醉對犬麻醉及鎮(zhèn)痛效果的評價

【摘要】:目前,應用于犬臨床手術中的麻醉方式主要有全身麻醉、局部麻醉,全身麻醉分為吸入麻醉和非吸入麻醉,單純非吸入性全身麻醉因其安全性及副作用,在臨床上單獨應用較少。局部麻醉因維持時間較短,麻醉效果不確實,均很少應用。復合麻醉因其起效快、麻醉穩(wěn)定,蘇醒時間短等優(yōu)點被廣泛地應用于犬的臨床研究中。以往的研究中復合麻醉主要采用吸入麻醉與靜脈麻醉復合的方式,尚不能達到較為理想的效果,而本研究采用靜脈麻醉與硬膜外麻醉復合的方法,為犬的復合麻醉提供一個新思路。 本研究采用丙泊酚復合硬膜外利多卡因的復合麻醉方式,通過對藥物的注射劑量及給藥時間進行摸索,并對麻醉效果,術后鎮(zhèn)痛效果及炎癥效果的各項指標進行監(jiān)測同時對該方法在犬臨床手術中的可行性進行了探討。本實驗以犬作為實驗動物按照麻醉方式不同分為兩組,Ⅰ組采用丙泊酚復合硬膜外利多卡因進行麻醉(丙泊酚麻醉誘導量6mg·kg-1,維持劑量0.4mg·kg-1·min-1,麻醉穩(wěn)定后,硬膜外注射6mg·kg-1的利多卡因);Ⅱ組作為對照組,采用靜脈連續(xù)輸注丙泊酚麻醉(丙泊酚麻醉誘導量6mg·kg-1,麻醉維持劑量0.71±0.6mg·kg-1·min-1),待麻醉穩(wěn)定后,分別進行腹部手術,并對比術后鎮(zhèn)痛及炎性效果以及麻醉犬的各項功能指標的變化。實驗結(jié)果如下結(jié)果表明: 與單純丙泊酚組比較,丙泊酚復合硬膜外麻醉組具有更好的麻醉效果,鎮(zhèn)痛、鎮(zhèn)靜和肌松效果良好;對生物反射影響較小,體溫(T)、心率(HR)、每分鐘呼吸數(shù)(RR)的變化平穩(wěn),都在生理范圍內(nèi);平均動脈壓(MAP)和血氧飽和度(SpO2)均表現(xiàn)出下降的趨勢,但其幅度較對照組;潮氣量(TV)、通氣量(MV)和呼吸頻率(RR)都下降,但是這些指標的變化都在犬的生理耐受范圍之內(nèi);與單純丙泊酚組比較,復合麻醉組的疼痛指標變化幅度較小,表現(xiàn)為P物質(zhì)(SP)、緩激肽(BK)、前列腺素(PG)、組胺(HIS)指標的變化為先升高后降低;炎性指標的變化表現(xiàn)為腫瘤壞死因子(TNF)、白介素-6(IL-6)、白介素-1(IL-1)、白介素-8(IL-8)的含量先增高后降低,且變化幅度較小,在犬的生理耐受范圍內(nèi)。 綜合結(jié)果表明,丙泊酚復合硬膜外麻醉組能達到較理想的麻醉及鎮(zhèn)痛效果,與單純輸注丙泊酚比較麻醉起效快、鎮(zhèn)靜、鎮(zhèn)痛、肌松效果較好、對呼吸的抑制作用較小,疼痛和炎性指標變化幅度較小,證明復合麻醉可應用于犬的腹部手術。
[Abstract]:At present, the main anesthetic methods used in canine clinical surgery are general anesthesia, local anesthesia, general anesthesia is divided into inhalation anesthesia and non-inhalation anesthesia, simple non-inhaled general anesthesia because of its safety and side effects. Local anesthesia is less effective and stable because of its short maintenance time and inaccurate anesthetic effect. The advantages of short recovery time have been widely used in the clinical study of dogs. In the previous studies, the compound anesthesia mainly adopted the combination of inhalation anesthesia and intravenous anesthesia, which could not achieve a more ideal effect. In this study, intravenous anesthesia and epidural anesthesia were used to provide a new idea for canine compound anesthesia. In this study, propofol combined with epidural lidocaine was used as a compound anaesthesia. The effect of postoperative analgesia and inflammation were monitored and the feasibility of this method in canine clinical operation was discussed.
