【摘要】：目的探討一期輸尿管雙J管置入聯(lián)合體外沖擊波碎石(extracorporeal shock wave lithotripsy,ESWL)治療嬰兒上尿路多發(fā)結(jié)石的安全性及有效性。方法 2013年1月~2015年6月我科對(duì)22例嬰兒上尿路多發(fā)結(jié)石行一期輸尿管雙J管置入聯(lián)合ESWL,全麻下經(jīng)F6或F8膀胱鏡置入輸尿管雙J管(F3或F4.6)后轉(zhuǎn)運(yùn)至碎石室,麻醉狀態(tài)下行ESWL(電壓7.5~10 k V,沖擊波頻率70次/min)。結(jié)果 22例手術(shù)均一期順利完成,術(shù)中無(wú)輸尿管穿孔、輸尿管黏膜撕脫發(fā)生,術(shù)后未發(fā)生泌尿系感染、尿源性膿毒血癥、腎被膜下血腫、輸尿管狹窄等嚴(yán)重并發(fā)癥。術(shù)后2~4周結(jié)石均完全排出(平均排石時(shí)間3.5周),排石率100%。術(shù)后隨訪(fǎng)觀察12個(gè)月,1例患兒術(shù)后1年復(fù)查發(fā)現(xiàn)腎結(jié)石復(fù)發(fā),行ESWL清除結(jié)石,其余無(wú)結(jié)石復(fù)發(fā),泌尿系超聲或CT未見(jiàn)腎臟及輸尿管形態(tài)改變,腎功能無(wú)異常。結(jié)論一期留置輸尿管雙J管聯(lián)合ESWL治療嬰兒上尿路多發(fā)結(jié)石,可有效減少手術(shù)次數(shù),減少腔內(nèi)侵入性有創(chuàng)操作,安全有效,值得臨床推廣。
[Abstract]:Objective to explore the issue of double J ureteral implantation combined with extracorporeal shock wave lithotripsy (extracorporeal shock wave lithotripsy, ESWL) in treating infantile upper urinary tract lithiasis of the safety and effectiveness of the method. In January 2013 ~2015 year in June in our department, 22 cases of infant urinary tract lithiasis underwent a double J ureteral implantation combined with ESWL after F6 or F8 tube, double J ureteral cystoscope implantation under general anesthesia (F3 or F4.6) after transfer to the stone manipulation room, anesthesia (ESWL 7.5~10 K V voltage, shock wave frequency 70 /min). The results of 22 cases of surgery were successfully completed, no ureteral perforation, ureteral mucosa avulsion, surgery no incidence of urinary tract infection, urinary derived sepsis, renal subcapsular hematoma, ureteral stenosis and other serious complications. 2~4 weeks after stones were completely discharged (mean expulsion time 3.5 weeks), the stone expulsion rate after 100%. 12 months of follow-up, 1 cases of postoperative 1 In the review found that the recurrence of kidney stone, ESWL removal of stones, no other changes of urinary calculi recurrence, ultrasound or CT no kidney and ureter form, renal function abnormalities.
