【摘要】：目的探討羅哌卡因復(fù)合芬太尼與布比卡因復(fù)合芬太尼在老年膝關(guān)節(jié)置換手術(shù)腰硬聯(lián)合麻醉患者中的應(yīng)用效果。方法將60例擇期行膝關(guān)節(jié)置換手術(shù)的老年患者隨機(jī)分為羅哌卡因復(fù)合芬太尼組(RF組)與布比卡因復(fù)合芬太尼組(BF組)。RF組:0.75%羅哌卡因2 ml+芬太尼1 ml(25μg),BF組:0.50%布比卡因2 ml+芬太尼1 ml(25μg)。觀察兩組感覺(jué)、運(yùn)動(dòng)阻滯效果,麻醉前后的血流動(dòng)力學(xué)變化及并發(fā)癥。結(jié)果 RF組感覺(jué)阻滯達(dá)到最高阻滯平面時(shí)間明顯長(zhǎng)于BF組(P0.01);RF組運(yùn)動(dòng)阻滯起效時(shí)間長(zhǎng)、恢復(fù)時(shí)間短,運(yùn)動(dòng)阻滯恢復(fù)時(shí)間較感覺(jué)阻滯恢復(fù)時(shí)間短,Bromage評(píng)級(jí)效果更好(均P0.05);BF組較RF組血流動(dòng)力學(xué)變化更明顯(P0.05)。結(jié)論羅哌卡因復(fù)合芬太尼在老年膝關(guān)節(jié)置換手術(shù)腰硬聯(lián)合中對(duì)血流動(dòng)力學(xué)影響小,感覺(jué)阻滯充分,運(yùn)動(dòng)阻滯恢復(fù)快。
[Abstract]:Objective to investigate the effect of ropivacaine combined with fentanyl and bupivacaine combined with fentanyl on spinal and epidural anesthesia in elderly patients undergoing knee arthroplasty. The rats were divided into ropivacaine plus fentanyl group (RF group) and bupivacaine plus fentanyl group (BF group: 0.75% ropivacaine 2 ml fentanyl 2 ml fentanyl 1 ml(25 / g BF group: 0. 50% bupivacaine 2 ml fentanyl 1 ml(25 渭 g). Results the maximum block time in RF group was longer than that in BF group (P 0.01) and the recovery time was shorter in RF group. The recovery time of motor block was shorter than that of sensory block. Bromage rating was better than that of sensory block. Conclusion ropivacaine combined with fentanyl in the spinal and epidural union of knee arthroplasty in elderly patients is more obvious than that in RF group. Hemodynamic effects are small, Sensory block is sufficient and motor block recovers quickly.
