【摘要】：背景與目的:以往研究顯示,術中輸注氨基酸可以緩解核心體溫的降低。該研究旨在探討術中輸注氨基酸對硬膜外阻滯復合全麻下行胃腸道腫瘤手術老年患者術后肝腎功能的影響。方法:選擇在硬膜外阻滯復合全麻下行胃腸道腫瘤手術的65~75歲患者40例,ASAⅠ~Ⅱ級,按隨機數(shù)字表法隨機分為2組:氨基酸干預組和0.9%氯化鈉溶液組,各20例。分別在麻醉誘導同時輸注11.4%復方氨基酸注射液2 m L·(kg·h)-1和0.9%氯化鈉注射液2 m L·(kg·h)-1直至手術結(jié)束。記錄誘導即刻、手術開始后90 min、關腹時鼻咽溫度;查術前、術后第1天、術后第1周的肝腎功能指標:總膽紅素(TBIL)、結(jié)合膽紅素(DBIL)、總蛋白(TP)、白蛋白(ALB)、谷丙轉(zhuǎn)氨酶(ALT)、谷草轉(zhuǎn)氨酶(AST)、尿素(BUN)、肌酐(Scr)、尿酸(UA)。結(jié)果:手術開始后90 min及關腹即刻,氨基酸干預組鼻咽溫度均高于0.9%氯化鈉溶液組,兩組比較差異有統(tǒng)計學意義(P0.05)。兩組各時間點肝腎功能指標均在正常范圍內(nèi)。兩組組內(nèi)比較,TBIL、DBIL在術后第1天均較術前升高(P0.05),但顯著低于術后第1周(P0.05);TP、ALB在術后第1天、術后第1周均較術前降低(P0.05);ALT、AST在術后第1天、術后第1周較術前升高(P0.05);BUN在術后第1天較術前降低(P0.05);術后第1周Scr、UA較術前及術后第1天降低(P0.05);UA在術后第1天較術前降低(P0.05)。但兩組間比較,差異均無統(tǒng)計學意義(P0.05)。結(jié)論:胃腸道腫瘤手術對老年患者術后肝腎功能有所影響,而術中輸注氨基酸在改善麻醉手術引起的低溫的同時不影響術后肝腎功能。
[Abstract]:Background and objective: Previous studies have demonstrated that intraoperative amino acid administration can alleviate transmission reduce the core body temperature. This study aimed to investigate the effects of amino acid infusion epidural anesthesia anesthesia on the gastrointestinal cancer surgery in elderly patients with postoperative liver and renal function during the operation.
