【摘要】：目的探討不同麻醉方式對(duì)老年腹部手術(shù)患者術(shù)后認(rèn)知功能障礙的影響。方法選取我院采用椎管內(nèi)麻醉方式進(jìn)行手術(shù)患者96例,作為觀察組;采用全身麻醉方法手術(shù)患者96例,作為對(duì)照組。比較兩組術(shù)中情況,采用簡(jiǎn)易精神狀態(tài)量表(MMSE)對(duì)麻醉前、麻醉后6 h、24 h、72 h患者精神狀態(tài)評(píng)分;并比較認(rèn)知功能障礙發(fā)生率。結(jié)果兩組患者術(shù)中血壓、心率、脈搏氧飽和度和手術(shù)時(shí)間差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),且觀察組麻醉時(shí)間略短于對(duì)照組(P0.01)。兩組患者術(shù)后不同時(shí)段MMSE評(píng)分均小于麻醉前(P0.05);觀察組患者術(shù)后6 h、24 h及72 h三個(gè)時(shí)間段MMSE評(píng)分均高于同期對(duì)照組(P0.01)。觀察組術(shù)后認(rèn)知障礙發(fā)生率為19.79%,低于對(duì)照組的33.33%(P0.05)。結(jié)論椎管內(nèi)麻醉在老年患者手術(shù)中使用安全可靠,術(shù)后認(rèn)知障礙發(fā)生率低,適宜于老年患者使用,值得臨床推廣應(yīng)用。
[Abstract]:Objective to investigate the effect of different anesthetic methods on postoperative cognitive dysfunction in elderly patients undergoing abdominal surgery. Methods 96 patients with spinal canal anesthesia and 96 patients with general anesthesia were selected as observation group. As the control group, the patients were assessed with MMSE before anesthesia and 24 hours after anesthesia, and the incidence of cognitive dysfunction was compared. Results the blood pressure and heart rate were compared between the two groups during operation. There was no significant difference in pulse oxygen saturation and operation time, and the anesthetic time in the observation group was a little shorter than that in the control group (P 0.01). The MMSE score of the two groups at different time after operation was lower than that before anesthesia, and that in the observation group was 24 hours and 72 hours after operation. The incidence of postoperative cognitive impairment in the observation group was 19.79, which was lower than that in the control group (33.33 and P 0.05). Conclusion Spinal anesthesia is safe and reliable in the elderly patients. The incidence of postoperative cognitive impairment is low and suitable for use in elderly patients.
