【摘要】：目的研究不同體位與發(fā)音對(duì)Mallampati氣道分級(jí)結(jié)果的影響及與改良Cormach-Lehane評(píng)分的相關(guān)性。方法行全麻下氣管插管手術(shù)的患者124例,ASAⅠ~Ⅱ級(jí),性別年齡不限。麻醉前對(duì)患者以不同體位及發(fā)音方式進(jìn)行Mallmpati氣道評(píng)估:仰臥位,頸部保持正中位置,最大程度張嘴、不伸舌;仰臥位,頸部保持正中位置,最大程度張嘴、伸舌;仰臥位,頸部最大程度后仰,最大程度張嘴、不伸舌;仰臥位,頸部最大程度后仰,最大程度張嘴、伸舌。每名患者先不發(fā)音,再發(fā)"啊"音,在手電筒光源下觀察咽部結(jié)構(gòu)并記錄分級(jí)情況。以相同的方式進(jìn)行麻醉誘導(dǎo),進(jìn)行改良Cormach-Lehane分級(jí)評(píng)估,記錄分級(jí)結(jié)果。計(jì)算Mallmpati評(píng)分與改良Cormach-Lehane分級(jí)的Spearman秩相關(guān)系數(shù)r,并計(jì)算各種評(píng)估方式的敏感性、特異性、陽(yáng)性預(yù)測(cè)值、陰性預(yù)測(cè)值及準(zhǔn)確性。結(jié)果仰臥位評(píng)估氣道時(shí),頸部后仰、最大程度伸舌、發(fā)"啊"音3種方式均能降低MMT評(píng)分,更充分暴露口咽部結(jié)構(gòu)。Mallampati評(píng)分與改良Cormach-Lehane無(wú)明顯相關(guān)關(guān)系,但是在仰臥位時(shí)頸部最大程度后仰、最大程度張嘴伸舌并發(fā)"啊"音評(píng)估Mallmpati評(píng)分時(shí)預(yù)測(cè)困難插管的敏感性、特異性、陽(yáng)性預(yù)測(cè)值、陰性預(yù)測(cè)值及準(zhǔn)確性最高。結(jié)論進(jìn)行仰臥位Mallampati氣道評(píng)估最理想的方式是頸部最大程度后仰、最大程度張嘴伸舌并發(fā)"啊"音。
